摘要
目的比较早期重症脑梗死患者肠内与肠外途径营养支持的营养指标及并发症,探讨两种营养支持途径的临床价值。方法回顾分析43例重症脑梗死的病例,其中22例采用胃肠道营养支持,21例采用胃肠外营养支持,比较两组患者营养支持后的血红蛋白、转铁蛋白、总蛋白、白蛋白等营养指标以及腹泻、腹胀、肠麻痹、胃肠道出血、肝功能损害、高血糖等并发症。结果两组在梗死后2周血红蛋白、转铁蛋白、总蛋白、白蛋白等营养指标差异无显著性,但胃肠外组腹泻、腹胀、肠麻痹、胃肠道出血、肝功能损害、高血糖的发生率明显高于胃肠道组,差异具有统计学意义。结论与肠外营养相比,早期肠内营养支持可较好地改善患者营养状况,且并发症少,是重症脑梗死患者较好的营养支持方式。
Objective To compare the nutritional index and complications of enteral and pareteral nutrition support in the severe cerebral infarction patients. Methods Review and analyse 43 patients who presented with severe cerebral infarction,in which 22 patients underwent enteral nutrition （group EN） and 21 pareteral nutrition （group PN） .The nutritional indexes such as HGB, TF, TP, ALB were compared and the complications such as diarrhea, windy, intestine paralysis, alimentary tract hemorrhage, hypohepatia and high blood glucose noted. Results The nutritional index between EN and PN group have no statistical difference, but the ratio of the complications in PN group are higher than that in EN. Conclusion Compared to parenteral, enteral nutrition support can improve the severe cerebral infarction patients＇ nutritional condition and reduce complications. Enteral nutrition support can be suggested as the better method.
