摘要
目的研究与卵巢癌相关的血清中恶性肿瘤相关物质（TSGF）、糖类抗原CA125、癌胚抗原（CEA）和甲胎蛋白（AFP）4种肿瘤标志的临床应用价值。方法采用生化比色法、AXSYM免疫分析仪分别检测48例卵巢癌患者、38例卵巢良性肿瘤及100例健康女性血清中的TSGF、CA125、CEA及AFP含量。结果单项肿瘤标志物检测，4种肿瘤标志水平均高于正常人和卵巢良性肿瘤患者，且差异均有统计学意义（P〈0．001）。TSGF敏感性最高达81．3％，特异性CA125最高达90．0％，与TSGF比较差异有统计学意义（x^2=7．79，P〈O．01），有效性CA125最高为85．8％。各种组合检测中，以TSGF＋CA125＋CEA＋AFP组获得的敏感性最高，为95．8％，但特异性和有效性较低，特异性和有效性CA125＋AFP为最高，但敏感性太低，仅为81．3％。综合考虑，以TSGF＋CA125＋AFP3项组合的敏感性、特异性、有效性和阳性预测值均较高。结论4种肿瘤标志物在卵巢癌诊断治疗中，具有一定的临床应用价值。TSGF＋CA125＋AFP组合后可明显提高卵巢癌诊断的敏感性和有效性。
Objective To investigate the clinical value of the serum tumor specific growth factor （TSGF）, CA125, carcinoembryonic antigen（CEA） and alpha fetoprotein（AFP） the in patients with ovarian cancer. Methods The contents of TSGF,CA125,CEA and AFP were analyzed in 48 patients with ovarian cancer,38 patients with benign ovarian disease and 100 healthy female adultsby Colorimetry of biochemistry and AXSYM analyzer. Results In the single examination, the 4 tumor markers in ovarian cancer were all higher than those in healthy adults and benign ovarian disease. There was a significant diference（P〈0.001）. The highest sensitivity of TSGF was 81.3 %. The highest specificity of CA125 was 90.0% with significant diference from TSGF. The highest effectiveness of CA125 was 85.80/oo. In all groups, the sensitivity of TSGF＋ CA125 ＋ CEA ＋ AFP was highest, which was 95.8%. The specificity and effectiveness were lower, which was the highest in the group of CA125＋ AFP, but sensitivity was lower （81.3%）. To sum up, the sensitivity, specificity, effectiveness and positive estimate value were all higher in the group of TSGF＋CA125＋AFP. Conclusion These four tumor markers have certain clinical application value for diagnosis and cure of Ovarian disease. The sensitivity and effectiveness can be increased with combined determination of TSGF＋ CA125＋AFP in diagnosis of ovarian disease.
