摘要
目的 评价16层螺旋CT仿真支气管内窥镜(VB)在中央型肺癌诊断中的应用。方法对56例经纤维支气管镜检查和病理证实的中央型肺癌患者进行16层螺旋CT仿真内窥镜检查。用16层螺旋CT机对患者进行扫描后,将数据建成CTVB图像进行分析,并与纤维支气管镜进行对照,其中30例与病理标本进行对照。结果CTVB显示100%的段以上支气管和90%以上的亚段支气管。肿瘤呈块状或结节状,引起管腔狭窄或闭塞。结论 评价16层螺旋CT仿真支气管内窥镜,酷似纤维支气管镜所见,能较好地辅助判断中央型肺癌的位置和支气管狭窄程度,结合其他三维重建功能,能同时显示管壁的厚度和肿瘤向管腔外侵犯的范围。
Objective To evaluate clinical applications of 16 - slice helical CT virtual bronchoscopy (CTVB) in the diagnosis of central lung cancer. Methods Fifty - six patients with central lung cancer ( all wereproved with fiberoptic bronchoscopy and pathology) underwent volume scanning using 16- slice helical CT( lightspeedpro -16). Data were transferred to computer workstation and CT virtual bronchoscopic imagings with pseudocolor encoding were generated using ticbronchoscopy,30 cases of all compared with navigator smooth software, and to compare with fiberopspecimen. Results 16- slice helical CTVB could reveal 100% segmental bronchus and 90% subsegmental bronchus simulating fiberoptic bronchoscopy. The tumors appeared as masses or nodules,causing bronchial wall thickening and occlusions. Conclusion 16 -slice helical CT virtual bronchoscopy simulates the images observed under fiberoptic bronchoscopy and can be used partly in lieu of it. h is helpful for identifying the location and de of bronchial stenosis in central lung cancer associated with muhiplanar reformat, 16 - slice helical CT virtual bronchoscopy can demonstrate extraluminal invasion of the tumor.
