摘要
目的了解不同程度心力衰竭患者心肌组织血管紧张素转换酶(angiotensin convering enzyme,ACE)和血管紧张素Ⅱ(angiotensinⅡ,AngⅡ)蛋白表达与心肌重构和心功能的关系。方法通过手术取材,采用Western blot技术检测35例瓣膜病所致心力衰竭患者和5例正常人的心肌组织中ACE和AngⅡ蛋白表达,采用免疫组织化学方法检测ACE和AngⅡ在心力衰竭患者和正常人心肌组织中的表达部位。结果瓣膜病所致心力衰竭患者心肌组织呈心肌重构病理改变。轻度心力衰竭患者心肌组织ACE和AngⅡ蛋白表达较正常人增高,中重度心力衰竭患者AngⅡ增高更为显著,但ACE/AngⅡ比值随着心力衰竭程度加重逐渐下降。免疫组织化学染色显示ACE主要在心肌细胞膜表达,AngⅡ主要在心肌细胞细胞质表达。结论心力衰竭患者心肌组织ACE和AngⅡ蛋白表达量明显增加,直接参与心肌重构的发生发展。心衰时ACE/AngⅡ值下降,提示AngⅡ的生成与分解可能依赖非ACE的其他途径。
Objective To explore the relationship of the expressions of myocardial angiotensin convering enzyme (ACE) and angiotensin Ⅱ (Ang Ⅱ ), and myocardial remodeling and cardiac functions in patients with congestive heart failure (CHF). Methods Pathologic and morphologic studies on myocardial tissue samples of 35 patients with CHF due to valvular heart diseases and of 5 matched health subjects dying of accidents were carried out with light and electronic microscopy assay. Protein expressions of ACE and Ang Ⅱ in myocardial tissues were analyzed using Western blotting. Immunohistochemistry was used to determine the positions of ACE and Ang Ⅱ expressions. Results Pathological changes of CHF patients showed typical myocardial remodeling. Their ACE and Ang Ⅱ expressions were increased than in the control subjects, but ACE/Ang Ⅱ ratio was decreased in the patients with severe CHF than in the mild CHF. Immunohistochemistry showed that ACE mainly expressed in the membrane of cardiocyte, and Ang Ⅱ mainly in the endochylema of cardiocyte. Conclusion The expressions of ACE and Ang Ⅱ are up-regulated in CHF patients, which may be involved in the myocardium remodeling. The ratio of ACE/Ang Ⅱ is decreased in CHF patients, suggested that Ang Ⅱ production may be ACE independent.
