摘要
目的调查分析多发伤患者并发MODS的危险因素、发病率和病死率。方法回顾性调查2001-01~2005-12收治上海长征医院急救科ICU的182例多发伤患者的临床资料,并将患者分为MODS组和非MODS组;采用SPSS12.0统计分析软件,以单因素分析中P<0.1的变量为自变量,MODS为因变量,作前进法二元(Binary)逐步Logistic回归分析,确定发生MODS的危险因素、各危险因素的相对危险度(RR)和95%可信区间(CI)。结果182例中存活153例,死亡29例,死亡率15.9%;MODS组48例,发生率26.4%,其中死亡20例,死亡率41.7%。单因素分析显示,进入Logistic回归分析的变量为年龄、ISS、休克、脓毒症、补液量和基础疾病等六项(P均<0.1),经过四步迭代,筛选出一个常数项和休克、年龄、ISS、脓毒症等四个变量进入回归方程,该方程预测是否发生MODS的总准确率为84.1%,此四个变量的P值分别为0.000、0.001、0.003和0.034,RR和95%CI分别为6.816(3.387~13.716)、3.381(1.694~6.744)、6.510(1.880~22.542)和2.608(1.077~6.316)。结论创伤失血性休克持续≥6h、年龄≥55岁、ISS≥20和脓毒症是多发伤并发MODS的危险因素。
Objective To investigate and analyze the morbidity, mortality and risk factors of multiple organ dysfunction syndrome (MODS) after multiple trauma. Methods The clinical data of all 182 patients with multiple trauma admitted to our ICU from January 2001 to December 2005 were retrospectively analyzed. The risk factors and their relative risk (RR) and 95% confidencial interval (CI) were determined with Binary Logistic Regression Analysis in SPSS 12.0. Results The morbidity of MODS among all 182 patients was 26.4% (48/182) , and the mortality was 41.7% (20/48). Binary Logistic Regression Analysis showed that four variables such as shock(durations〉6 hours) , patient's age ≥ 55, injury severity score(ISS) 〉120 and the presence of sepsis were in the equation, and their RR and 95% CI were 6. 816(3.387 -13.716) , 3.381(1.694 -6.744) , 6.510(1.880 -22.542) and 2.608 (1.077 -6.316) ,respectively. Conclusions Shock(duration≥6 hours) , patient's age≥55, injury severity score(ISS) ≥20 and the presence of sepsis are risk factors of MODS after multiple trauma.
