摘要
目的探讨食管鳞癌组织中缺氧诱导因子-1α(HIF-1α)和survivin的表达及其临床意义。方法采用免疫组织化学SP法检测50例食管鳞癌组织中HIF-1α和survivin蛋白表达,分析蛋白表达与临床病理特征及预后的关系。结果HIF-1α、survivin蛋白阳性表达率分别为68%和72%,两者存在正相关关系(γ=0.623,P<0.001)。HIF-1α、survivin表达与食管鳞癌临床分期、远处转移密切相关(P<0.05),HIF-1α的表达还与淋巴结转移有关(P<0.05);两者表达均与病理类型、病变长度无显著相关性(P>0.05)。HIF-1α表达阳性和阴性组放疗后完全缓解率及中位生存期分别为8.8%、10个月和81.3%、25个月,两组比较有显著性差异(P≤0.001);1、2、3年生存率分别为38.2%、6.0%、2.9%和81.3%、54.2%、7.9%,Log-Rank检验两组生存曲线有非常显著差异(P=0.001)。结论食管癌组织中存在HIF-1α、survivin蛋白表达,两者表达呈正相关关系,HIF-1α表达与食管鳞癌的放射抗拒性密切相关。
Objective: To explore the expressions of hypoxia inducible factor-1α (HIF-1α)and survivin in esophageal squamous cell cancer (ESCC) and their clinical significance. Methods: The expression of HIF-1α and survivin in 50 patients with ESCC who received radiotherapy was deteced by immunohistoehemical SP method. The relationship between protein expressions and clinicopathological features and prognosis were analyzed. Results: The positive rates of HIF-1α and survivin protein expression were 68% and 72% , respectively. There was a positive relationship between them ( γ = 0. 623, P 〈 0. 001 ). The expressions of HIF-1α and survivin were closely related with clinical stage and distant metastasis. The expression of HIF-1α correlated with lymph node metastasis. The expressions of HIF-1α and survivin did not correlate with the degree of differentiation and the length of the primary tumor ( P 〉 0. 05 ). The complete effective rate and mean survival period after radiotherapy were 8.8% and 10 months and 81.3 % and 25 months in HIF-1α-positive and HIF-1α-negative group, respectively (P =0.000). The 1-, 2-, 3-year survival rates were 38.2%, 6.0% and 2.9% and 81.3%, 54.2%, and 7.9% in HIF-1α-positive and HIF-1α-negative group, respectively. Log-rank test suggested that the difference was significant between HIF-1α-positive and HIF-1α-negative group (P=0. 001 ). Conclusion: HIF-1α and survivin were overexpressed in ESCC. The expression of HIF-1α protein positively correlates with expression of survivin protein. The expression of HIF-1α has a close relationship with radiotherapy resistance of ESCC.
