摘要
目的探讨经尿道等离子体双极电切术治疗浅表膀胱肿瘤（PKRBt）的安全性与有效性。方法用英国Gyms公司的经尿道等离子体双极电切系统行浅表膀胱肿瘤切除术160例。肿瘤分级G1为121例，G2为39例。患者男性118例，女性42例，年龄31～89岁，平均59．7岁。多发肿瘤41例，单发119例，膀胱侧壁有肿瘤分布者53例。结果经尿道膀胱肿瘤等离子体双极电切术手术时间5～48min，平均（16±14）min，无经尿道电切综合征；53例侧壁肿瘤切除时，28例发生闭孔神经反射，2例导致各l处膀胱穿孔。随访1～12个月，17例复发，8例再行PKRBt，9例改行根治术。结论PKRBt是一种安全、有效的手术方式。闭孔神经反射是PKRBt的常见并发症之一。
Objective To assess the efficacy and safety of transurethral resection of superficial bladder tumor with plasmakinefic （PKRBt）. Methods 160 patients with superficial transitional cell carcinoma （121 G1 and 39 G2 ） were treated by PKRBt. There were male 118 and female 42 between 31 and 89 years old （mean 59. 7 years）. 53 with tumors appeared at the lateral wall. All patients were followed - up in 1 - 12 months postoperatively. Results The mean duration of the procedure was 16 ＋ 14 min （5 to 48 min） with little bleeding. No TUR syndrome was found. The adductor reflex was found in 28 cases with turnor located at the lateral wall and bladder perforation occurred in 2 patients. Conclusion Transurethral resection of superficial bladder tumor with plasmakinetic is effective and safe. The adductor reflex is one of the common complications of PKRBt.
