摘要
目的研究冠心病支架植入术前、术后不同时间相sCD40L水平变化,以探讨sCD40L与再狭窄的可能关系。方法选择接受普通支架置入术的冠心病患者41例(研究组),健康正常对照组16例。分别于术前、术后1,5,15,180d取外周静脉血测定血清sCD40L。所有患者随访6个月。结果冠心病患者术前血清sCD40L为(6.54±1.55)ng/ml,明显高于对照组的(2.11±0.68)ng/ml(P<0.05)。支架内再狭窄率24.4%(10/41)。再狭窄患者支架术前sCD40L水平为(7.34±1.25)ng/ml,明显高于无再狭窄患者的(4.36±0.98)ng/ml;术后血清sCD40L水亦显著高于无再狭窄患者(P均<0.01);再狭窄患者支架术后高水平sCD40L持续至术后6月,而无再狭窄患者术后5d则恢复至正常。结论再狭窄患者支架术前、术后血清sCD40L水平增加提示sCD40L可能与支架内再狭窄有关。
Objective To investigate the serial changes in circulating concentrations of soluble CD40 ligand (sCD40L) and its relation to restenosis before and after coronary stent implantation in patients with coronary heart disease. Methods We studied 41 patients with coronary heart disease who underwent successful stent implantation, and 16 healthy volunteers were studied as a control group. Serum sCD40L was measured before and 1,5,15,and 180 days after stent implantation. The patient follow-up was 6 months. Results Patients with coronary heart disease had preprocedural sCD40L (6. 54 ± 1.55 ) ng/ml significantly higher than control group (2.11 ± 0.68) ng/ml, P 〈 0.05. Restenosis occurred in 10 patients (24.4%). Restenotic patients had preprocedural sCD40L(7.34± 1.25 ) ng/ml and postprocedural sCD40L significantly higher than nonrestenotic patients preprocedural (4.36 ±0.98) ng/ml,P 〈 0. 01. Enhanced sCD40L after stent implantation persisted as statically significant throughout the study in the restenotic patients, whereas they normalized 5 days after the procedure in nonstenotic patients. Conclusions Restenosis patients had significantly higher preprocedural and postprocedural sCD401L, which suggests that sCD40L may he associated with in-stent restenosis.
