摘要
目的探讨团体人际心理治疗(IPT)方案治疗产后抑郁(PPD)患者的效果。方法对23例确诊为产后抑郁患者,采用团体人际心理治疗。总治疗次数12次,治疗前、后及治疗后第6个月随访用爱丁堡产后抑郁量表(EPDS-10)、汉密尔顿抑郁量表(HAMD-17)、人际间问题问卷(IIP)和夫妻适应量表(DAS)评估。结果(1)23例患者治疗后抑郁均分显著低于治疗前(EPDS-10:t=5.10,P<0.001;HAMD-17:t=6.40,P<0.001;治疗前、后DAS和IIP的得分比较差异无统计学意义);(2)14例痊愈患者与9例未愈(部分缓解+无缓解)患者治疗前抑郁评分(EPDS-10和HAMD-17)差异无统计学意义,但对该两组患者治疗前IIP的社交回避、复仇2个子量表比较结果显示:痊愈患者较少回避社交(t=3.50,P<0.01)和较少复仇(t=2.80,P<0.01);(3)23例产后抑郁患者治疗后第6个月抑郁分显著低于治疗前评分(EPDS-10:F=24.30,P<0.001;q=6.44,P<0.01;HAMD-17:F=33.59,P<0.001;q=4.88,P<0.01)。上述时点IIP和DAS总分比较无显著性改变。结论团体人际心理治疗方案对产后抑郁有较好的治疗效应。
Objective To study the efficacy of group interpersonal psychotherapy （IPT） for the patients with postpartum depression （PPD）. Methods 23 patients diagnosed with PPD according to DSM-IV diagnostic research criteria were intervened with IPT for 12 times totally and assessed by using EPDS-10, HAMD-17, liP, DAS at pre-treatment, post-treatment and the sixth month follow-up. Results （1）Mean score comparisons revealed a significant change from baseline before intervention in both EPDS-10 and HAMD-17 scores （EPDS-10： t=5.10, P〈0.01; HAMD-17： t=6.40, P〈0.001）. However, there were no significant differences on the IIP or the DAS total scores. （2） The difference was non-significant in the scores of EPDS-10 and HAMD-17 between the 14 cured cases and 9 uncured cares （partly relieved or no relieved） before intervention, but the study on pre-interventional factors of social obviation and re- venge in liP showed the cured patients behaved with less social obviation and revenge （t=3.50, P〈0.01; t=2.80, P〈0.01）; （3） At the sixth month after intervention, all of them scored at an obvious lower level than concerning depression before intervention （EPDS-10： F=24.30, P〈0.001; q=6.44, P〈0.01： HAMD-17： F=33.59, P〈0.001; q=4.88, P〈0.01）, but no change in participant＇s liP total scores or their DAS scores across the three time points. Conclusion Group IPT approach is of better efficacy in reducing postpartum de- pressive symptoms.
