床旁脑电图对昏迷病人的预后评价 被引量:4

Diagnostic Value of Bedside Electroencephalogram to Coma Patients' Prognosis in Intensive Care Unit

摘要 目的探讨床旁脑电图(EEG)对评价昏迷病人脑功能及预后判断的价值。方法采用便携式床旁脑电图监测仪对GCS评分≤8分,入院3d的70例昏迷病人进行床旁脑电图监测,使用三种EEG分级标准对所测EEG结果进行分级、对比,分析其与患者预后的相关性。结果Lavizzari、Synek和Young三种脑电图分级标准与预后均有较好的相关性,相关系数分别为0.640,0.657,0.595,三种脑电图分级标准比较,Synek标准预测的准确性、敏感性和特异性分别为84.4%,86.7%和82.4%,均较其他标准为高。结论床旁脑电图监测能对昏迷病人的预后提供良好的诊断价值,Synek标准较其他两种其预测价值更高。 Objective To review the clinical diagnostic value of bedside Electroencephalogram (EEG) to the prognosis of coma patients in Intensive Care Unit (ICU) and compare the value of different EEG standard for the evaluation of prognosis. Methods 70 patients recruited 3 days in ICU with brain function serious illness , GCS ≤ 8 were selected. Mobile EEG was monitored to all of the 70 patients . The data was analysised by salty doctor and divided different grade according to three EEG standald (Lavizzari , Synek and Young standard). The association of EEG grades and the prognostic was reviewed. Results Lavizzari、Synek and Young EEG grades had good correlation to the prognosis. The correlation coefficient was 0.640,0.657 and 0.595,respectively. Synek grade had the best correlation coefficient, sensitivity, specificity and accuratissime. Conclusion Bedside EEG has good value to the prognosis of coma patients in ICU . Synek grade is the best one in the three EEG diagnosis grades.
