摘要
目的:总结近10年来对脉搏波传导速度及脉搏波形分析在动脉弹性功能检测方面的研究成果,探讨它们在糖尿病患者动脉弹性功能检测中的意义。资料来源:应用计算机检索Medline1991-01/2006-12关于脉搏波传导速度,脉搏波形分析与糖尿病患者动脉弹性关系的文章,检索词“diabetes mellitus,arterial elasticity,pulse wave velocity,pulse wave analysis”,并限定文章的语种为“English”;同时检索中国期刊全文数据库2002-01/2006-09的相关文章,限定文章的语种为中文,检索词“糖尿病,动脉弹性,脉搏波传导速度,脉搏波形分析”。资料选择:对资料进行初审,选择动脉弹性功能检测、心血管危险因素与动脉弹性、脉搏波传导速度与糖尿病,脉搏波形分析与糖尿病等相关文献。排除重复性研究。资料提炼:共收集到相关文献78篇,33篇符合上述标准。其中动脉弹性功能检测相关文献5篇,心血管危险因素与动脉弹性相关文献13篇,脉搏波传导速度与糖尿病相关文献7篇,脉搏波形分析与糖尿病相关文献7篇、糖尿病与冠状动脉粥样硬化性心脏病相关文献1篇。资料综合:糖尿病是重要的心血管危险因素之一,动脉弹性可反映心血管危险因素所致的血管功能损伤。脉搏波传导速度和脉搏波形分析为无创性的反映动脉弹性功能改变的方法,可发现糖尿病患者亚临床的血管病变。文章中还具体介绍了脉搏波传导速度和脉搏波形分析的原理、方法和临床意义。结论:脉搏波传导速度和脉搏波形分析可能在糖尿病血管并发症的早期发现,在危险分层及预后判断中发挥重要作用。
OBJECTIVE: To summarize new achievements on applying pulse wave velocity and pulse wave analysis to measure arterial elasticity in recent 10 years, and probe into the significance of applying them to measure arterial elasticity in patients with diabetes mellitus, DATA SOURCES: The relevant articles of the relationship between pulse wave velocity, pulse wave analysis and arterial elasticity in patients with diabetes mellitus from January 1991 to December 2006 were searched for in Medline by imputing the key words of "diabetes mellitus, arterial elasticity, pulse wave velocity, pulse wave analysis" in English into computer. Similarly, the relevant Chinese articles between January 2002 and September 2006 were retrieved in China Journal Full-text Database (CJFD) with the key word of "diabetes mellitus, arterial elasticity, pulse wave velocity, pulse wave analysis". STUDY SELECTION: Data were checked in the first trial, and literatures related to arterial elasticity function measurement, risk factors of cardiovascular diseases and arterial elasticity, pulse wave velocity and diabetes mellitus, pulse wave analysis and diabetes mellitus were selected, Repetitive studies were excluded. DATA EXTRACTION: A total of 78 relevant articles were collected, out of which 33 articles were in accordance with above-mentioned criteria. In 33 articles, 5 articles were related to arterial elasticity function measurement, 13 articles were related to risk factors of cardiovascular diseases and arterial elasticity, 7 articles were related to pulse wave velocity and diabetes mellitus, 7 articles were related to pulse wave analysis and diabetes mellitus, and 1 article was related to diabetes mellitus and coronary atherosclerotic heart disease, DATA SYNTHESIS: Diabetes mellitus is one of the important risk factors of cardiovascular diseases. Arterial elasticity can reflect the lesion of blood vessel function resulting from risk factors of cardiovascular diseases. Pulse wave velocity and pulse wave analysis are noninvasive examinations to reflect the changes of arterial elasticity function,we can find the subclinical vascular disease in patients with diabetes mellitus by using them. This paper also introduces the mechanism, method and clinical significance of pulse wave velocity and pulse wave analysis, CONCLUSION: Pulse wave velocity and pulse wave analysis play important roles in early finding, risk stratification and prognostic judgment of vascular complication in patients with diabetes mellitus.
