气管插管后气囊上液与呼吸机相关性肺炎的相关性 被引量:15

Correlation of ventilator associated pneumonia and liquid on air sac after endotracheal intubation

摘要 目的探讨气管插管后气囊上液与呼吸机相关性肺炎(VAP)的相关性。方法对气管插管后确诊为VAP的患者,进行气囊上液、下呼吸道、口腔分泌物、胃液培养,并进行菌群对比,找出其相关性。结果下呼吸道分泌物与气囊上液的细菌成分为正相关,差异具有统计学意义,P<0.05。结论气囊上液是引起气管插管患者的呼吸机相关性肺炎的主要原因之一,气囊上液的细菌大多数来源于胃腔和口腔细菌的定植。 Objective To investigate the relativity between liquid on air sac and ventilator associated pneumonia(VAP) after endotracheal intubation. Methods To cultivate bacteria from the liquid on air sac,lower respiratory tract, oral and stomach cavi- ty,compare and analyse the relationship to VAP. Results There was positive correlation of the bacteria of lower respiratory tract and liquid on air sac, P 〈 0.05. Conclusion The liquid on air sac is one of the main reasons to cause the VAP. Bacteria of the liquid on air sac are mostly from invasion of stomach and oral cavity.
