摘要
目的探讨Visfatin基因在肥胖者腹部皮下和网膜脂肪组织中的表达情况及相关因素。方法根据体质指数(BMI)的不同,将40例胆石症择期手术患者分为肥胖组20例,对照组20例。用RT-PCR方法检测两组的大网膜与皮下脂肪组织Visfatin mRNA的表达水平,并测定血糖(FBG)、空腹胰岛素(FINS)、血脂和胰岛素抵抗指数(HOMA-IR)。结果①肥胖组和对照组网膜组织的Visfatin mRNA表达量均高于对应的皮下脂肪组织(均P〈0.01)。Visfatin mRNA表达量肥胖组网膜高于对照组(P〈0.01);皮下脂肪Visfatin mRNA表达量肥胖组与对照组比较差异无统计学意义(P〉0.05)。②合并2组数据,网膜脂肪组织Visfatin mRNA表达量与血甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、极低密度脂蛋白胆固醇(VLDL-C)呈正相关(均P〈0.05),与其他因素均无相关性(均P〉0.05);腹部皮下脂肪组织Visfatin mRNA表达量则与总胆固醇(TC)、TG、VLDL-C具有相关性(均P〈0.05)。③多元逐步回归分析发现,皮下脂肪Visfatin mRNA表达量与舒张压(DBP)和年龄相关(均P〈0.05);网膜Visfatin mRNA表达量与VLDL相关(P〈0.05)。结论网膜Visfatin表达量可能与肥胖发生及血脂代谢相关。
Objective To study the expression of Visfatin mRNA in abdominal subcutaneous and omental adipose tissue and related factors in obese subjects. Methods 40 patients of cholelithiasis were divided into obese group and normal group ( n = 20 for each) according to BMI. The expression of visfatin mRNA in abdominal subcutaneous and omental adipose tissue was measured using RT-PCR and fasting blood glucose ( FBG ) , fasting insulin (FINS) ,blood lipid profiles and HOMA-IR were measured. Results ①The visfatin mRNA expression revels in omental adipose tissue were significantly increased in obese group and normal group than those in subcutaneous tieeue respectively (P 〈 0.01 ). The visfatin mRNA expression in omental adipose tissue was significantly increased in obese group compared with normal group( P 〈 0.01 ) ,but decreased subcutaneous adipose tissue ( P 〉 0.05 ). ②The expression level of visfatin mRNA in all omental adipose tissue was positively correlated with TG, HDL-C, and VLDL ( P 〈 0.05 ) , and was not correlated with other factors(P 〉 0.05 ) ; the expression level of visfatin mRNA in subcutaneous adipose tissue was correlated with TC,TG, and VLDL(P 〈 0.05 ). ③Multi-stepwise regression analysis demonstrated that the expression level of visfatin mRNA in omental adipose tissue was significantly correlated with VLDL( P 〈0.05 ) ,the expression level of visfatin mRNA in abdominal subcutaneous adipose tissue was correlated with DBP and age ( P 〈 0.05 ). Conclusion The level of visfatin mRNA in omental adipose tissue may be of an important parameter of obesity and a correlated factor in lipid metabolism.
