152株鲍曼不动杆菌耐药性分析 被引量:17

摘要 目的了解鲍曼不动杆菌的临床分布及其耐药谱。方法回顾性分析某院2004—2005年医院感染鲍曼不动杆菌的分布。用头孢硝噻吩对鲍曼不动杆菌进行β-内酰胺酶检测,采用琼脂扩散法进行药敏试验。结果152株鲍曼不动杆菌主要分布在重症监护室(ICU,31株)、烧伤科(29株)、神经内科(19株);其中产β-内酰胺酶菌株85株,产酶率为55.92%;对氨苄西林、头孢哌酮、哌拉西林、阿莫西林/克拉维酸、头孢他啶、环丙沙星的耐药率>50.00%,对氨苄西林/舒巴坦、头孢哌酮/舒巴坦、美罗培南的耐药率分别为15.13%、11.18%、12.50%,具有较好的抗菌活性。结论医院感染鲍曼不动杆菌主要分布在ICU、烧伤科、神经内科等科室,具有多重耐药性,临床应加强监测并根据药敏结果合理选用抗菌药物治疗。 Objective To investigate clinical distribution and antimicrobial resistance of Acinetobacter baurnannii. Methods Distribution of Acinetobacter baurnannii isolated from patients with nosocomial infection in a hospital from 2004 - 2005 was analysed retrospectively. β-lactamases produced by Acinetobacter baurnannii were detected by nitrocefin, antimicrobial susceptibility tests were performed by Kirby-Bauer method. Results One hundred and fiftytwo Acinetobacter baurnannii were mainly distributed in intensive care unit(31 strains), burn unit(29 strains) and neurology department(19 strains) ; 85 (55.92%) of which were β-lactamases-producing strains, the resistant rates to ampicillin, cefoperazone, piperacillin, amoxicillin/clavulanic, ceftazidime and ciprofloxacin were all 〉50. 00%, the resistant rates to ampicillin/sulbactam, cefoperazone/sulbactam and meropenem was 15. 13%, 11. 18% and 12. 50% respectively. Conclusion Acinetobacter baurnannii strains mainly distribute in intensive care unit, bum unit and neurology department, antimicrobial agents should be chosen according to antimicrobial susceptibility test results.
