影响医源性股动脉假性动脉瘤压迫性治疗预后的相关因素分析 被引量:6

摘要 目的探讨影响介入术后股动脉假性动脉瘤(假瘤)压迫性治疗预后的相关因素,使临床医生更合理地把握治疗时机和方法。方法全部20例经CDFI确诊,行局部加压包扎或超声引导下加压修复治疗。测量记录瘤体大小、瘤颈流速、瘤颈类型及附壁血栓,并监测各指标在治疗过程中的变化。结果本组中两种压迫治疗方法治愈率无显著性差异。瘤体小、瘤颈流速偏低、瘤颈细长以及治疗过程中瘤颈处流速逐渐减低、瘤体不继续增大、瘤腔内附壁血栓逐渐增多是预后好的征象。下肢静脉血栓是压迫性治疗中最易发生的并发症。结论超声提供的信息有助于判断假瘤的预后和治疗转归。两种压迫方法效果相近,各有特点,应当根据情况合理应用,在假瘤压迫性治疗中,应加强下肢静脉血栓的预防。 Objective To discuss the relative factors affecting the prognoses of compression therapy in the postcatheterization femoral arterial pseudoaneurysm (PSA). Methods All 20 cases were diagnosed by CDFI and treated with local compressive bandaging treatment or ultrasound-guided compression and repair(UGCR). In all cases, the size, the flow velocity at the neck, type of the neck, mural thrombosis of PSA were measured and monitored during the treatment. Results The two treatments were of no significant differences. The PSA being small size, low flow velocity at the neck and gradually decreasing, long and thin neck, stopping augmenting and having more and more mural thrombosis during the treatment were good signs. The common complication of compression treatment was the deep vein thrombosis. Conclusion The information's offered by CDFI are helpful to evaluate the prognoses and guide the treatment of PSA. The two treatments all have good efficacy,considering whose differrent charercterizations, to choose the proper one according to the circumstances,and be careful the deep vein thrombosis during the compression treatment.
