摘要
目的了解以上腹部不适就诊的14岁以下患儿幽门螺杆菌(Hp)的感染情况,探讨应用免疫印迹法分型诊断儿童Hp感染的临床意义。方法采取705例以上腹部不适就诊患儿的静脉血,应用免疫印迹法进行Hp抗体谱带分析,分型诊断Hp感染。当检出有与Hp细胞毒相关蛋白A(CagA)和(或)空泡毒蛋白A(VacA)相关的抗128、116、95、和91kD蛋白抗体时,Hp的感染类型定为I型;仅有与Hp尿素酶和鞭毛蛋白相关的抗30、66、50和54kD蛋白抗体时,Hp的感染类型定为II型。结果共检出Hp抗体阳性468例,总阳性率为66.38%,其中I型Hp占44.87%,II型Hp占55.13%。共有63例患儿经内镜检查明确诊断患有消化性溃疡,其中的Hp抗体总阳性率为84.54%,I型Hp抗体总阳性率为82.54%,比未被诊断为消化性溃疡患儿的64.50%和24.61%均显著增高。结论利用免疫印迹法可方便地对儿童Hp感染进行血清学分型诊断;Hp感染在腹部不适的患儿中较为普遍,尤其是在消化性溃疡患儿中;I型Hp感染与消化溃疡的关系更为密切,应进行根除治疗。
[Objective] To investigate the infectious rate of Hehcobacter pylori (Hp) in children who complained discomfort in upper abdomen at least for 3 months, and to discuss significance of using Western blot method to serotype Hp infection. [Methods] 705 vein blood samples taken from the children complaining of discomfort in upper abdomen were tested by Western blot to analyze anti-Hp-IgG bands. Presence of any antibodies to 128, 116, 95, and 91 kD Hp proteins related with CagA and VacA, was defined as type Ⅰ Hp infection; while presence of only antibodies to 30, 66, 50 and 54 kD Hp proteins related with urease and flagellin, was defined as type Ⅱ Hp infection. [Results] 468 cases were found to be positive by one of Hp antibodies, Hp infectious rate in the children was66.38%, in which 44.87% was type Ⅰ and 55.13% was type Ⅱ. 63 cases of children were diagnosed peptic ulcers by endoscopy, of whom Hp infectious rate was 84.54% and the type Ⅰ Hp infection rate was 82.54%, which were significantly higher than Hp infectious rate 64.50% and type Ⅰ Hp infection rate 24.61% in the children without peptic ulcer. [Conclusion] Hp infection could be serotyped expediently by Western blot; the infection of Hp was prevalent in the children complaining of discomfort in upper abdomen, especially in the children with peptic ulcer;, the type Ⅰ Hp had relationship with peptic ulcers more closely and should be eradicated.
