Analysis on clinical characteristics of primary hepatic carcinoma

摘要 目的了解原发性肝癌(PHC)临床特点,为PHC的防治提供依据。方法采用回顾性调查方法,对310例PHC住院患者的临床资料进行分析。结果80%以上的患者以非特异性症状与体征就诊,12.26%因并发症、合并症或转移症状就诊,3.55%因体检异常就诊;在有病理诊断的病例中,肝细胞癌占70.7%(29/41);HBV感染率为81.61%,男性高于女性(P<0.01);AFP阳性率为70.5%,HBV感染者AFP阳性率明显高于非HBV感染者(P<0.01);2.9%具有血吸虫病史,33.23%有嗜烟、酒史;4.52%具有PHC家族史。结论HBV感染是PHC的主要病原学因素,嗜酒、吸烟及血吸虫病等也可能是肝癌的风险因素。积极预防和控制HBV感染流行,是减少PHC发生的关键所在;定期对健康人群尤其是高危人群进行B超、AFP等检测,早发现、早治疗是提高原发性肝癌生存率的关键。 Objective To understand the clinical characteristics of primary hepatic carcinoma (PHC) and provide the basis for the prevention and treatment of PHC. Methods By using the method of retrospective study, the clinical characteristics of 310 PHC cases in hospital were statistically analyzed. Results Above 80% visited by the non-specific symptom and physical signs, 12.26% visited by complications or other illness, 3. 55% visited by the abnormality of health examination. Among the cases which had pathological diagnosis, hepatic cellular cancer accounted for 70.7%. The infective rate of HBV was 81. 61%, the male was higher than the female (P〈0. 01). The positive rate of AFP was 70.5%. 33.23% patients had smoking and drinking alcohol, 4.52% patients had family history of PHC. Conclusion The HBV infection is the main etiology factor of PHC. Smoking, drinking alcohol may also be the risk factor of PHC. Actively preventing and controlling HBV prevalence is the key to decrease the occurrence of PHC. Regularly earring on type-B ultrasonic and detection of AFP to the healthy crowd, in particular those in high-risk group, early discovery and treatment are the key to improve the survival rate of patients with PHC.
