大肠腺癌组织中生存素基因的表达及临床意义 被引量:1

Expression and clinical significance of survivin gene in large intestinal adenocarcinomas

摘要 目的检测大肠正常黏膜、大肠腺瘤和大肠腺癌组织中生存素的表达,探讨其与大肠癌临床病理特征的关系。方法采用组织芯片、免疫组织化学和计算机图像分析技术检测22例大肠正常黏膜、62例大肠腺瘤和80例大肠腺癌组织中生存素的表达。结果生存素表达率在大肠正常黏膜为0.0%,腺瘤为38.7%,腺癌为58.8%,三组间的表达差异有显著性意义(P<0.01);生存素在高分化腺癌表达率为42.9%,明显低于低分化腺癌73.1%(P<0.05),在Dukes'分期、淋巴结转移等组间比较差异无显著性意义(P>0.05)。生存素阳性表达者5年生存率(48.00%)与阴性者5年生存率(80.00%)相比显著降低(P<0.05),且生存素具有独立的预后意义。结论生存素在大肠肿瘤中过度表达可能参与大肠肿瘤的形成和发展,生存素过度表达可作为大肠癌患者预后不良的独立指标。 Objective To investigate the relationship between the expression of survivin and clinicopathological indices in large intestinal tumors. Methods The expression of survivin was detected by immunohistochemical methods and image analysis in 22 cases of large intestinal normal mucosas, 62 cases of large intestinal adenomas and 80 cases of large intestinal adenocarcinomas. Results The positive percentages of survivin expression showed an adscendent tendency from normal large intestinal mucosas (0.0%) to large intestinal adenomas (38.7%) and to large intestinal adenocarcinomas (58.8%). There were significant differences among them (P 〈0.05). The positive percentages of survivin expression were related to histological differentiation grade (P 〈0.05), but not related to lymph node metastasiS, and Dukes' stage of large intestinal adenocarcinomas (P 〉0.05). The 5-year survival percentages were lower in patients with survivin-positive adeno- carcinomas (48.00%) than in those with survivin-negative adenocarcinomas (80.00%). Multinle Cox regression analyses demonstrated that survivin expression was an independent prognosticator (P 〈0.05), Conclusions Survivin is over-expressed in large intestinal tumors. The upregulated expression of survivin may play a positive role in the oncogenesis and progression of large intestinal tumors. The survivin expression may be an independent prognosticator for patients with large intestinal adenocarcinomas.
