摘要
目的报道应用逆行岛状皮瓣修复肢体远端皮肤软组织缺损的临床效果。方法临床应用12种不同类型的逆行岛状皮瓣修复肢体远端皮肤软组织缺损共160例。皮瓣类型:虎口背侧筋膜蒂皮瓣25例,指动脉逆行岛状皮瓣10例,指背筋膜蒂皮瓣9例,掌背动脉逆行岛状皮瓣8例,尺动脉腕上皮支皮瓣23例,前臂骨间背动脉岛状皮瓣28例,桡动脉鼻烟窝皮支皮瓣15例,桡浅神经营养血管皮瓣10例,腓肠神经营养血管皮瓣12例,胫后动脉内踝上皮支皮瓣12例,足底内侧逆行皮瓣3例,足背动脉逆行皮瓣5例。其中血管蒂皮瓣104例;筋膜蒂皮瓣34例;神经营养血管皮瓣22例。结果145例逆行岛状皮瓣完全成活,皮瓣完全坏死9例,皮瓣远端部分坏死6例。术后随访,成活的皮瓣外观、质地良好。结论逆行岛状皮瓣具有操作简单、成功率高且皮瓣较薄,与受区皮肤质地、色泽比较接近等优点,是修复肢体远端皮肤软组织缺损较理想的选择。
Objective To report the outcome of repairing defect of the soft tissue in the distal limb with reverse-flow island flap. Methods One hundred and sixty cases with defect of the soft tissue in the distal limb were repaired by 12 kinds of reverse-flow island flap. Flap type： reversed fascial flaps on dorsal of the first web 25 cases; reversed island flaps pedicle of digital artery 10 cases; reversed fascial island flaps on dorsal of the finger 9 cases; the dorsal metacarpal flaps 8 cases; ulnar super carpal cutaneous artery flaps 23 cases; dorsal interossous artery island flaps 28 cases; surf-box flaps 15 cases; reversed nero-fasciocutaneous flaps pedicled on the superficial radial nerve 10 cases; sural neurovascular flaps 12 cases; supramalleolar medial cutaneous artery flaps of posterior artery 12 cases; reversed medial plantar flaps 3 cases ; reverse-flow island flaps pedicle of the dorsal artery of foot 5 cases. Among of them, the island flaps pedicle of artery 104 cases; the island flaps pedicle of fascia 34 cases; neurovascular flaps 22 cases. Results One hundred and forty-five reverse-flow flaps survived completely, total necrosis 9 cases, 6 cases appeared partial necrosis on the distal of the flap. Follow-up period ranged from 3 to 12 months postoperatively, both appearance and texture of the surviving flaps were excellent. Conclusion Reverse-flow island flap is easy to operate, survival rate higher and it＇ s texture and colour is similar to skin on the distal limb. It is suitable for clinical doctors to repair the soft tissue defect in the distal limb.
