64层3D-CTA对颅内动脉瘤诊断及临床评估的应用价值 被引量:12

Value of 64-slice 3D-CTA in diagnosis and surgical treatment of intracranial aneurysm

摘要 目的通过比较64层3D-CTA与DSA在临床诊断颅内动脉瘤及其评估外科治疗的优缺点,评价64层3D-CTA在颅内动脉瘤诊断及治疗中的临床应用价值。方法回顾性分析2005年9月至2006年12月期间,重庆医科大学附属第一医院神经外科99例自发性蛛网膜下腔出血患者的临床资料,患者均行3D-CTA检查,分析CTA检查结果并与DSA检查结果比较。结果99例SAH患者经3D-CTA检查发现动脉瘤70例,其中41例同时接受DSA检查,除1例外,全部证实DSA的发现, DSA所漏诊的1例经手术证实为微小动脉瘤。结论64层螺旋CTA对颅内动脉瘤的诊断敏感性及特异性高,且能够清楚显示解剖结构,有助于治疗方法的选择和难度的评估。 Objective In order to evaluate the clinic value of 3D-CTA, we compared 64-slice 3D- CTA with DSA in the diagnosis and surgery treatment of intraeranial aneurysm. Methods Clinical records of 99 consecutive SAH patients admitted from September 2005 to December 2006 were reviewed. All of them were examined by 3D-CTA. To evaluate the clinic value of 3D-CTA, we adopt DSA or operative findings as the evaluation eriterion. Results In 99 patients, 70 eases with intraeranial aneurysm was found on CTA. Among the 70 aneurysm patients, 41 eases underwent DSA that confirmed the CTA findings in 40 eases. One ease who was missed by DSA was confirmed by surgery. Conclusions 3D-CTA is a sensitive and specific modality for deteeting and eharaeterizing aneurysms and ean demonstrate elearly the relative anatomy that is useful in option of manipulation and evaluation of diffieuhies in treatment of aneurysms.
