摘要
目的分析肿瘤病人院内感染因素,找出应对措施,降低感染发生率。方法根据2006年1月到12月我科1433例出院病人的院内感染发生情况,分析相关感染因素,制定防范措施,降低院内感染发生率。结果1433例住院肿瘤患者中感染42例,占2.9%,其中下呼吸道感染21例,上呼吸道感染7例,胃肠道感染7例,皮肤感染1例,血管相关感染2例,其他4例。结论化疗病人并发症多,血象低,口腔黏膜溃疡的发生,药物外渗引起的皮肤血管炎症等等是引起院内感染的因素。另外,各种损伤性检查、治疗的开展,各种留置导管的使用及抗生素的不合理应用,消毒隔离,无菌技术的规范执行不严,还有医疗废弃物的处理不当都给院内感染提供了机会。因此我们认为必须加强医务人员相关知识培训,严格执行院内感染规范,注意医院环境管理,控制院内感染发生率。
Objective To investigate the factors associated with nosocomial infection in cancer patients and the solutions. Methods Data of nosecomial infection in 1433 patients discharged in 2006 were analysed. Results Infection developed in 42 patients, with infection rate being 2. 9%. There were 7 upper respiratory tract infection, 21 lower respiratory tract infection, 7 gastrointestinal infections, 1 skin infection, 2 vascular infection and 4 other infections. Conclusion There are many precipitating factors for infections in chemotherapy patients, such as chemotherapy complications, low white blood cells, oral mucositis and ulcer, extravasation of drugs. Besides, invasive examination and treatment, indwelling catheters, improper use of antibiotics increase the chance of infection. Therefore, medical personnel should become acquainted with infection prevention, implement strictly aseptic techniques, pay attention to hospital environmental management.
