摘要美国、英国等国为应对人口的老龄化，实现控制公共医疗开支，同时保证健康服务的质量，不断强调出院计划（discharge planning）、连续护理（continuum of care）等概念及其对有效利用卫生资源的重要意义。为出院病人制定并实施有效出院计划（discharge planning）的重要意义已经得到医学护理工作者和不少发达国家政府健康主管部门的认可和重视。
