巩膜宽带环扎术后眼球三维形态的测量 被引量:2

摘要 目的探讨视网膜脱离巩膜宽带环扎术对眼球三维尺寸和容积的影响及屈光的改变。方法行巩膜宽带环扎术的孔源性视网膜脱离26例,于术前、术后1周测量A型超声、角膜地形图,以磁共振成像(MRI)技术测量13例患者眼球的尺寸和容积。结果巩膜宽带环扎术后眼轴延长,前房变浅,晶状体增厚,玻璃体腔加深;角膜平均屈光力及散光无显著改变;MRI显示术后眼球和玻璃体腔容积显著减少,前房容积和晶状体体积无显著改变;环扎处眼球的水平内径、垂直内径、周长显著减小。结论巩膜宽带环扎术减少了玻璃体腔容积,促使视网膜平复;同时通过眼球壁代偿的眼轴后延使近视增加。 Objective There is no report of 3-dimensional change following retinal detachment surgery. The purpose of this article was to assess the changes of volume and dimensions of eyeball as well as changes of refractive status following scleral broad encircling surgery. Methods Twenty-six eyes from 26 patients with rhegmatogenous retinal detachment received successful broad-band scleral encircling surgery from July,2002 to December,2003 in Second Hospital of Wenzhou Medical College. Axial length (AXL) , depth of anterior chamber (ACD) , lens thickness (LENS) and vitreous depth were examined by A-scan ultrasonography. Corneal topography was performed for Simkl , Simk2 and astigmatism examinations. Volume of eyeballs and dimensions along the three cardinal axes in 13 eyes were determined with Gyroscan MRI system. All the measurements above were performed before operation and the 1st week after operation. Results The AXL, ACD, LENS and vitreous depth were significantly enhanced in postoperation compared with preoperation ( P = 0.03 ,P = 0. 01 , P = 0.01 ,P = 0. 003, respectively). The corneal power and astigmatism had a insignificant increase in postoperation ( P = 0. 16, P = 0. 66 ). Volume of the eyeball and volume of vitreous decreased by( 24.13 ± 6. 95 )% (P = 0.01 ). Horizontal and vertical dimensions of eyeball were significant decreased after operation compared with before operation ( P 〈 0. 01 ). The circumference decreased by ( 33. 11 ± 8.69 ) % in postoperation. Conclusion Scleral encircling surgery results in alteration of eyeball parameters. The buckle inducing an indentation on sclera and loss of vitreous is the main cause of these changes. MRI has an important role in 3-dimension measurement of retinal attachment surgery.
