摘要
目的探讨儿童喉乳头状瘤手术的麻醉方法。方法对我院近年收治的28例喉乳头状瘤患儿的术中麻醉资料进行分析。结果在静脉复合麻醉下,大部分患儿术中生命体征平稳,放置支撑喉镜顺利,安全度过围手术期,仅少数患儿因并发喉痉挛或喉水肿导致呼吸道梗阻行气管插管或气管切开。结论儿童喉乳头瘤手术的麻醉关键是保证有效通气的情况下,保持足够的麻醉深度,使声带松弛无咽喉反射,心血管反应轻,术毕苏醒快,避免任何可加重喉梗阻和呼吸困难的因素。
Objective To explore the anesthesia methods used in the operation of pediatric laryngeal papilloma. Methods The anesthesia records of the 28 children, suffering from laryngeal papilloma who were cured in our hospital recently, were analyzed. Results Most of the children were placed pedestal laryngoscope successfully and passed the perioperative period safely. The patients could keep stable vital signs. A few of them were exerted tracheal intubation and incision of trachea because of respiratory obstruction induced by laryngospasm and laryngeal edema. Conclusion To keep an effective ventilation is the key point of anesthesia during the operation of pediatric laryngeal papilloma. It is important to keep suitable depth of anesthesia so as to loosen the vocal band, avoid laryngeal reflex, lighten cardiovascular effects and wake up quickly after operation. Any factors which can aggravate the laryngeal obstruction and dyspnea should be avoided too.
