异丙酚复合瑞芬太尼或氯胺酮微泵法行小儿静脉全麻的临床观察 被引量:7

摘要 目的:探讨异丙酚复合瑞芬太尼或氯胺酮微泵法行小儿非插管静脉全麻的效果及其对患儿呼吸、循环和苏醒时间的影响。方法:60例手术患儿,随机分为两组,Ⅰ组为"异丙酚-瑞芬太尼"合剂,Ⅱ组为"异丙酚-氯胺酮"合剂,术中监测MAP、HR、RR、SpO2,定时查动脉血气。结果:两组术中镇痛均完善,Ⅰ组患儿安静,分泌物少,停药后5~10min苏醒;MAP、HR在T1、T2时点有不同程度的下降(P<0.05),T3略有上升(P>0.05);RR、SpO2和动脉血气无明显变化(P>0.05)。Ⅱ组患儿基本安静,分泌物较多,停药后10~30min苏醒;MAP、HR有不同程度的上升(P>0.05);RR、SpO2和动脉血气无明显变化(P>0.05)。结论:异丙酚复合瑞芬太尼或氯胺酮微泵法行小儿静脉全麻给药恒速且用量少、镇静、镇痛完善,对呼吸、循环影响轻微,均可安全用于小儿非插管全凭静脉麻醉。尤其以异丙酚复合瑞芬太尼可控性强、并发症少、苏醒快且彻底,优于异丙酚复合氯胺酮。
