摘要
目的在C型臂的引导下,经皮椎体成形术治疗骨质疏松性椎体压缩骨折,探讨该技术的临床疗效及应用价值。方法15例骨质疏松性椎体压缩骨折患者22个椎体,在C型臂X线透视下,取俯卧位,经皮穿刺,在病椎注入骨水泥。术前及术后3d进行疼痛视觉类比评分(VAS评分)、活动能力评分,并进行统计学处理。术前、术后40层螺旋CT扫描,观察椎体形态改变。结果治疗后15例患者疼痛较治疗前有明显好转,VAS评分差异(P<0.001)有统计学意义;活动能力也有不同程度改善,活动能力评分治疗前后差异(P<0.001)有统计学意义;部分椎体的前缘和椎体中部高度有一定恢复。结论经皮椎体成形术治疗骨质疏松性椎体压缩骨折止痛效果明显,能很快改善患者的活动能力,早期下床活动。该手术属微创手术,在骨科临床有一定的应用前景。
Objective To evaluate the therapeutic effect of percutaneous vertebroplasty (PVP) guided by C-arm fluoroscopy, in treating osteoporotic vertebral compression fractures. Methods Fifteen patients with 22 vertebral compression fractures, bone marrow biopsy needle was inserted, under the guidance of C-arm fluoroscopy. Then polymethymethactrylate (PMMA) was injected into the fractured vertebra. Visual analogue scale (VAS) and score of mobility were evaluated, 3 days before and after PVP treatment. Shape of vertebra was observed by MSCT, pre- and post of treatment. Results After PVP, fifteen patients' pain was relieved considerably, with a significant difference( P 〈 0.001 ) in VAS; The score of patient's mobility sharply increased ( P 〈 0.001 ) ; The shape of some vertebra was retrieved slightly. Conclusions PVP is an effective mini-invasive technique for treatment of osteoporotic vertebral compression fractures. It has a strong effect on anti-pain and can improve patient's mobility. So PVP has its orooer orosoect in orthopaedies clinic.
