摘要
目的观察应用腓肠肌内侧头肌皮瓣转移治疗胫骨中上段创伤性慢性骨髓炎合并骨缺损骨外露的疗效。方法对27例胫骨慢性创伤性骨髓炎合并骨缺损骨外露的患者,采用病灶清除,Ⅰ期植骨,外固定支架固定,腓肠肌内侧头肌皮瓣转移的方法进行治疗。结果本组27例均得到1 a以上随访,所有转移皮瓣全部成活,除2例骨髓炎复发、植骨失败外,其余病例炎症均未复发,植骨愈合好,植骨愈合时间2.7-6.3个月,平均3.7个月。结论腓肠肌内侧头肌皮瓣转移治疗胫骨中上段创伤性慢性骨髓炎并骨外露骨缺损,可以提高疗效,缩短病程,减少费用,是一种切实可行的有效方法。
Objective It is to observe the effect of repair of traumatic tibia osteomyelitis in middle-superior segment combined with osseous defect and expose by transfering medial head of gastrocnemius musculocutaneous flap. Methods 27 cases of patients with traumatic tibia osteomyelitis in middle and upper part combined with osseous defect and expose were treated with debridement, bone grafting, external fixation, and transfering of medial head of gastrocnemius musculocutaneous flap. Results The 27 cases were followed up over one year, all the medial head of gastrocnemius musculocutaneous flaps kept alive, Osteomyelitis recurrences and osseous nonunion occurred in only two cases. To union the reconstructed bone cost 2.7 - 6 months, mean 3.7 months. Conclusion This method could improve the therapeutic effect, shorten the course of illness, decrease the treatment cost, so it is a feasible and effective method for the treatment of traumatic tibia osteomyelitis combined with osseous defect and expose.
