摘要
目的探讨合适剂量的多沙普仑用于静吸复合全麻后既有较好催醒效果,又能保持生命体征平稳及减少不良反应。方法120例静吸复合全麻病人随机分A、B、C、D四组,全麻后催醒多沙普仑分别为0.2mg/kg、0.5mg/kg、1.0mg/kg和1.5mg/kg。记录用药前,用药后2min、5min、10min、15min血压、心率、呼吸次数、潮气量等生命体征数据;观察神志恢复情况及不良反应。结果A组B组患者血压、心率用药后较用药前有不同程度上升,但无显著性差异(P>0.05),C组D组患者血压、心率用药后较用药前明显上升(P<0.05);A组患者呼吸次数、潮气量用药后较用药前有不同程度上升,但无显著性差异(P>0.05),B组C组D组患者呼吸次数、潮气量用药后较用药前明显增加(P<0.05);C组、D组各有5例和9例出现恶心、出汗、肌肉抽搐、烦躁等不良反应,D组1例病人出现惊厥。B、C、D组2min、5min、10min、15min清醒率明显高于A组(P<0.05),C、D组2min、5min、10min、15min清醒率与B组比较无显著性差异(P>0.05)。结论全麻后静注多沙普仑0.5mg/kg有较好的催醒和呼吸抑制逆转作用,副作用的发生也较少,是较理想的催醒药物剂量。
Objective To find the most appropriate dosage of Doxapram Hydrochloride used in arousing patients after general anesthesia. Methods 120 patients who would undergo general anesthesia were divided into four groups randomly. After anaethesia, Doxapram Hydrochloride was used, the dosages of them were 0.2mg/kg, 0.5 mg/kg, 1.0mg/kg and 1.5mg/kg.The vital signs, the degrees of wakefulness and side-effectssuch as convulsion, headache, sweating, nausea and itching were recorded. Results After treatments the blood pressures and heart rate increased very little (P〉0.05) in groupA and B, and the increments in group C and D were much stronger than that in group A and B at 2th min,5th min, 10th min and 15th min (P〈0.05);Some side-effects such as nausea, sweating, itching, and fidgets were found in 5 patient of group C and 9 patients of group D. There was 1 patients of group D was found convulsing. After the treatment with Doxapram Hydrochloride the rates of awake in group B, C and D at the time of 2th rain and 5th were much higher than that in group A (P〈0.05). Condusions 0.5mg/kg Doxapram Hydrochlorlde injected via veins is good for patients who have undergone general anaesthesia.
