摘要
目的:探讨健步走对糖耐量减低患者血清抵抗素、胰岛素敏感性的影响。方法:对41名糖耐量减低(IGT)患者随机分为健步走组20例、对照组21例,健步走组采取24周健步走运动处方锻炼。运动干预前后测定身体成分、抵抗素(Resistin)游离脂肪酸、空腹血糖、OGTT2h血糖和空腹胰岛素,计算胰岛素抵抗指数(HOMA-IR)。结果:1)健步走组经过24周运动干预后,除空腹血糖外,BMI、体脂%、腰围、腰臀比、OGTT2h血糖、空腹胰岛素、HOMA-IR、抵抗素、游离脂肪酸较试验前及对照组显著下降;2)对照组试验前后各项指标无明显变化;3)多元逐步回归分析显示,运动干预前后HOMA-IR的变化量成为影响运动干预前后抵抗素变化量的重要因素。结论:采用中等强度规律的健步走运动干预可减低抵抗素水平,提高胰岛素敏感性,改善糖代谢。
Objective: to explore the effects of walking on the serum resistin and insulin sensitivity in patients with impaired glucose tolerance (IGT). Method: Forty-one patients with IGT were divided randomly into exercise group and control group.The exercise group members took an aerobic exercise prescription (walking) for 24 weeks and the control group without additional exercise as usual. Before and after the research, body composition, level of resistin, free fatty acid , fasting blood glucose, 2-hour postprandial blood glucose and fasting insulin were examined. Meanwhile, the homeostasis model assessment-insulin resistant index (HOMA-IR) was calculated. Results: (1) In the exercise group, body mass index (BMI), body fat percentage (BF%), waist circumference, waist to hip ratio (WHR), 2-hour postprandial blood glu- cose, fasting insulin, HOMA-IR, resistin and the content of free fatty acid decreased significantly; (2) Each index in the control group was not changed significantly; (3) The multiple regression analysis indicated that the variables influenced the level of resistin included BMI, the content of free fatty acid and insulin. Conclusion: The exercise prescription of medium tenacity walking could reduce the level of resistin and improve the insulin sensitivity and blood glucose metabolism.
