脐带绕颈新生儿的脐动静脉血气及差值的分析 被引量:8

摘要 目的:研究脐动静脉血气及其差值与脐带绕颈新生儿缺氧的关系。方法:对40例脐带绕颈新生儿和40例正常新生儿的脐动、静脉血进行血气分析,测定pH、PCO2、PO2、HCO3-、BE、HCT和脐动静脉血气差值[Δ(A-V)],并对两组的指标进行比较。结果:脐带绕颈组的脐动脉pH低于对照组,而PCO2高于对照组(P<0.05);两组脐静脉的各项指标差异无显著性;脐带绕颈组的脐动静脉差值ΔpH、ΔPCO2均大于对照组(P<0.05),而ΔPO2、ΔHCO3-、ΔBE、ΔHCT差异无显著性。结论:脐带绕颈加重分娩时胎儿的缺氧状态,脐动静脉血气差值可反映脐带绕颈新生儿的缺氧程度,未发现脐带绕颈导致胎儿贫血。 Objective.To investigate the anoxic state of newborns with nuchal cords by comparing the umbilical arterial and venous blood gases and their differences between them.Methods:Umbilical artery and umbilical venous blood samples from the full-term newloom of normal control group (n=40) and nuchal cords group (n=40) were collected. PH, PCO2, PO2, HCO3, BE and hematocdt were measured. The differences of these parameters between umbilical artery and vein △(A-V)were calculated. Results: The pH, PCO2, PO2, HCO3, and BE obtained from umbilical vein of beth groups were not statistically different, The pH of umbilical artery in nuchal group was significantly lower than that of the control group while the PCO2 was higher. Arteriovenous △(A-V) pH and PCO2 differences in the nuchal group were greater than those of the control group. No significance was found in arteriovenous PO2, HCO3, BE, and hematocrict differences.Conclusions: Neonates with umbilical cord around neck is thought to aggravate the anoxic state of newlooms when delivering, which can be reflected by umbilical arteriovenous blood gas differences. Nuchal cord doesn' t made newborn anemia.
