摘要
目的探讨食管癌中基质金属蛋白酶2(MMP-2)表达与临床病理因素间的关系及其对单纯放疗预后的影响。方法应用免疫组化法检测59例食管癌和41例食管癌旁组织中MMP-2表达的情况,分析该蛋白表达与临床病理因素间的关系及其对食管癌单纯放疗预后的影响。结果59例食管癌组织中MMP-2阳性表达率为59.32%,41例癌旁组织中阳性表达率为31.71%,两者间差异有统计学意义(P=0.007);胸部CT扫描显示病变最大浸润深度、气管和(或)支气管受侵、主动脉受侵及椎前三角受侵等与MMP-2表达均有关系。MMP-2表达对食管癌单纯放疗的预后有显著影响。结论MMP-2高表达与食管癌进展有关,很可能促进肿瘤侵袭转移,可作为食管癌放疗预后的一个评价指标。
Objective To investigate the differential expression of matrix metalloproteinase-2 （MMP-2） and prognostic factors in esophageal squamous cell carcinoma treated by radiotherapy. Methods MMP-2 protein expression was measured in 59 esophageal carcinoma and 41 peri-carcinoma tissues with immunohistochemistry, the relationship between the protein expression and the clinicopathological parameters, and the prognostic factors in esophageal squamous cell carcinoma after purely radiotherapy were analyzed. Results The rates of positive expression of MMP-2 in esophageal carcinoma and the peri-carcinoma tissues were 59.32 % and 31.71%, the difference between two groups was significant（ P =0. 007）. The expression of MMP-2 was significantly correlated with the depth of tumor invasion in CT image, the infiltration of trachea and bronchi, the infiltration of aorta, the status of fat interspace between vertebra, the metastasis of lymph node of neck and mediastinal lymph node. The expression of MMP-2 had significant correlation on prognosis of esophageal carcinoma. Conclusion The expression of MMP-2 is correlated with tumor invasion, it probably plays an important role in tumor invasion,and is one of the prognostic factor in esophageal carcinoma treated by radiotherapy.
