摘要
目的检测增殖性糖尿病视网膜病变(proliferative diabetk-retinopathy,PDR)患者玻璃体中血管内皮生长因子(vascular endothelial growth factor,VEGF)及色素上皮衍生因子(pigment epithelium-derived factor,PEDF)水平,了解玻璃体手术后糖尿病性视网膜病变的进展是否与术前 VEGF 及 PEDF 水平有关。方法采集行玻璃体切除术患者58例(60眼)的玻璃体样本,其中 PDR 患者28例(30眼),孔源性视网膜脱离患者30例(30眼),用酶联免疫吸附法检测玻璃体中 VEGF 及 PEDF 水平,随访观察玻璃体手术后糖尿病性视网膜病变的进展情况。结果 PDR 组玻璃体中的 VEGF 及 PEDF 浓度分别为(4187.21±316.35)pg/mL 和(25.01±11.85)μg/mL,高于孔源性视网膜脱离组的(3949.93±441.94)pg/mL 和(17.06±10.16)μg/mL(P<0.05)。PDR 组在随访中失访7例,糖尿病性视网膜病变进展患者5例(6眼),糖尿病性视网膜病变无进展患者16例(17眼),PDR 组中进展和无进展患者玻璃体中 VEGF 及 PEDF 浓度的差异无统计学意义(P=0.889;P=0.624)。结论 VEGF 及 PEDF 可能共同参与调节增殖性糖尿病视网膜病变的形成,但并不支持下调 PEDF 作为这种调节机制的假设。PDR 患者术前玻璃体中 VEGF 及 PEDF 浓度尚不能作为预测玻璃体手术后糖尿病性视网膜病变是否进展的依据。(中国眼耳鼻喉科杂志,2007,7:347-349)
Purpose To measure the levels of vascular endothelial growth factor(VEGF) and pigment epithelium-derived factor (PEDF) in the vitreous of patients with proliferative diabetic retinopathy(PDR). To study the relationship between the preoperative levels of VEGF and PEDF in the vitreous and the postoperative progression in diabetic retinopathy(DR). Methods Vitreous samples were collected from 60 eyes of 58 patients who underwent vitrectomy for the treatment of PDR(30 eyes of 28 patients) or rhegmatogenous retinal detachment (30 eyes of 30 patients). The levels of VEGF and PEDF in these vitreous samples were determined by enzyme-linked immunosorbent assay. The postoperative progression of DR was observed. Results The mean vitreal VEGF and PEDF level was (4 187.21± 316.35) μg/mL and (25.01 ±11.85 )μg,/mL in eyes with PDR respectively, which were significantly higher than (3 949.93 ± 441.94) μg/mL and ( 17.06± 10.16) μg/mL in eyes with rhegmatogenous retinal detachment ( P 〈 0.05 ). Seven cases were out of follow- up in the PDR group. In DR group, progression was seen in 6 eyes of 5 patients, nor in 17 eyes of 16 patients. No significant difference in the vitreal VEGF and PEDF was found between patients with and without progression( P =0.889 and P = 0. 624). Conclusions Both VEGF and PEDF might modulate the development of PDR, but the results did not support the hypothesis that downregulation of PEDF as a mechanism for the modulation. The preoperative levels of VEGF and PEDF in the vitreous can't be the basis for the prediction of postoperative progression in DR.
