术中超声引导下颅内囊性血管母细胞瘤切除新术式的应用 被引量:2

Value of Intraoperative Ultrasound Assisted in Intracranial Cystic Hemangioblastoma Surgical Treatment

摘要 目的:评价术中超声实时导航下颅内囊性血管母细胞瘤手术切除新术式的价值。材料和方法:搜集2005年10月至2006年10月间共10例经术前影像学检查诊断为囊性血管母细胞瘤的患者。使用Aloka SS D4000型超声检查仪,配备数据外科专用小凸弧探头。常规开颅暴露硬脑膜后,利用超声导航确定囊性血管母细胞瘤实质性瘤结节的位置,以此决定手术入路,先切除瘤结节,再剥离囊壁,完整切除肿瘤,以病理为标准,并影像随访1年。结果:10例小脑囊性血管母细胞瘤术中都迅速准确地找到了实质性瘤结节,术后病理证实肿瘤完全切除,术后随访1年未见复发。结论:术中超声导航下小脑囊性血管母细胞瘤切除新术式不仅具有一般术中超声缩短手术时间,减少对正常组织损伤的优势,而且能够保障瘤体的完整切除,减少复发。 Purpose: To discuss the value of real - time intraoperative ultrasound(IOUS) guidance technique applied in the surgery procedure of intracranial cystic hemangioblastoma. Materials and Methods: Patients: from octorber, 2005 to oetorber, 2006, 10 consecutive patients diagnosed as cystic hemangioblastoma were studied.All of them were then received IOUS assisted surgical resection in Huashan Hospital. Equipment: an Aloka SSD4000 ultrasound system equipped with a convex intraoperative probe was used. Methods: after routine eraniotomy, the dura mater was exposed. IOUS technique was then used to locate the mural nodule, and the surgical approach was decided based on this location. The mural nodule was first cut, and then the cystic wall was peeling off. We use pathological result as the golden standard, all the patients was performed radiology examination within one year. Results: 10 hemangioblastoma was all located in cerebellum. With our improved surgical procedure, all the tumors could be quickly located intraoperatively. Post - operative pathology results confirmed the total resection of the tumor, and no recurrence case was reported within one year following - up.Conclusion: The combination of IOUS and the modified surgical approach used in this article has not only the advantage of general IOUS: significantly shortened surgical time and spared the surrounding normal tissues, but also can confirm the thorough removed of tumor, reduce the rate of recurrence.
