代谢综合征与早期肾功能损害的关系 被引量:7

Relationship between metabolic syndrome and early-stage renal dysfunction

摘要 目的:探讨代谢综合征(MS)与早期肾功能损害发生的相关性。方法:选择2004年8月至2006年12月期间152例在我院心血管内科住院MS患者和100例健康体检者进行资料统计。结果:(1)MS组24小时尿微量白蛋白(MA)明显高于正常组(P<0.01),内生肌酐清除率(Ccr)显著低于正常组(P<0.01);(2)存在不同数量代谢因子成分MS各组间肾功能指标比较MA、Ccr、血肌酐(Cr)均无统计学差异(P>0.05),而不同代谢因子组合MS患者肾功能指标比较,MA存在统计学差异(P<0.05),高血压合并高血糖MS患者MA高于单纯高血压MS组或高血糖MS组(P<0.05),各组间Ccr、Cr无统计学差异(P>0.05)。(3)Logistic多元回归分析表明:高血糖、高血压、血脂异常、胰岛素抵抗均为影响早期肾功能损害发生的独立危险因素,OR值分别为7.961,7.234,1.325,1.241(P<0.01或P<0.05)。结论:MS患者易存在早期肾功能损害。高血压合并高血糖MS患者MA明显高于单纯高血压MS患者或高血糖MS组患者。高血糖、高血压、血脂异常、胰岛素抵抗均为影响早期肾功能损害发生的独立危险因素,但是高血糖、高血压为最重要的危险因素。 Objective: To study the relationship between metabolic syndrome(MS) and early-stage renal dysfunction. Methods: A total of 152 cases of MS and 100 cases of health control were screened for earlystage renal dysfunction, and some correlated risk factors were analyzed. Results: (1) Quantitation of 24h urine protein in MS group was significantly higher than the control (P 〈0.01 ) ,but the creatinine clearance rate (Ccr) was significantly lower in MS group( P 〈 0.01 ). (2)Exist different each set of MS kidney function index sign comparison MA, Ccr of the factor composition of the amount metabolism, blood creatinine all had no covariance to learn difference ( P 〉 0.05 ), but dissimilarity metabolism factor combination MS sufferer the kidney function index sign comparison, the MA existence shows covariance to learn difference(P 〈 0.05 ), the high blood pressure complicated high blood sugar MS sufferer MA was higher than pure high blood pressure MS set or high blood sugar MS set( P 〈 0.05 ), each Ccr, Cr had no significant difference( P 〉 0.05 ). Logistic analysis showed that hyperglycemia, high blood pressure , blood fat abnormity, insulin resistance were all independent risk factors of early-stage renal dysfunction, each OR value was 7. 961,7. 234,1. 325,1. 241 (P 〈0.01 or P 〈0.05 ). Conclusion: The sufferer existed the renal dysfunction in earlier period. The high blood pressure complicated high blood sugar MS sufferer MA was obviously higher than the sufferer of high blood sugar MS set or in the pure high blood pressure MS sufferer. Hyperglycemia, high blood pressure, blood fat abnormity, insulin resistance were all independent risk factors of early-stage renal dysfunction. Hyperglycemia and high blood pressure were the two most important independent risk factors.
