摘要
目的报道示指全长脱套伤一期修复临床效果。方法一期重建示指全长套状撕脱缺损3例。伤指中远节以躅甲瓣或第二足趾修复，伤指近节以躅甲瓣或第二足趾携带的跖背和跖底皮瓣包裹背侧、桡侧和掌侧，伤指近节尺侧面以跖背皮瓣富余的筋膜瓣覆盖后，采用皮片移植修复。结果经5～58个月．平均38个月随访。临床修复3例均完全成活，2例效果满意，另1例因淋巴水肿，外形较臃肿。结论应用跗甲瓣或第二足趾携带跖背和跖底皮瓣移植，与皮片移植结合，一期修复全长脱套伤示指，可获预期的临床效果。
Objective To report the clinical outcome of the repair of degloved avultion of whole length index finger in one stage. Methods The whole length index finger with degloved avuhion were reconstructed in 3 cases. The distal and middle phalange as well as the radial,dorsal and palmar surfaces of proximal phalanx were reconstructed by the wrap around flap of big toe or the second toe with the flap from the dorsum and planta of the metatarsals,and the ulnar surfaces of proximal phalanx were covered by skin graft. Results Three patients were followed-up for an average of 38 months（range：5 to 58 months）. Reconstructed index fingers are survived ,and the function and cosmetic outcome are good but one is relatively bulky for lymphedema. Conclusion The methods for reconstruction of whole length index finger with degloved avuhion are effective in this series.
