摘要
目的探讨2型糖尿病患者眼底血流动力学变化与糖尿病视网膜病变发生、发展的关系。方法应用彩色多普勒超声对41例(82只眼)糖尿病患者眼动脉、视网膜中央动脉(CRA)、睫状后动脉(PCA)进行检测,并与正常组进行对照。全部病例均经裂隙灯、眼底镜和荧光血管造影检查,并将2型糖尿病患者的眼底分为非增殖期(NPDR)、增殖期(PDR)和未见视网膜病变(NDR)三组。结果2型糖尿病患者眼动脉、CRA、PCA的收缩期最大血流速度(PSV)、舒张末期血流速度(EDV)和平均速度(VM)均降低,阻力指数增高。其中NPDR、NDR组各值与对照组比较差异有统计学意义(P<0.05),而PDR组各值与对照组比较差异有非常显著性意义(P<0.01)。结论糖尿病患者眼血流动力学改变与视网膜病变的发生、发展有密切关系,是影响视网膜病变的重要因素。彩色多普勒超声对眼血流动力学的检测能为临床了解糖尿病视网膜灌注情况及功能变化提供重要信息,对评价糖尿病视网膜病变的程度有着重要价值。
Objective To investigate the correlation between the ocular hemodynamic changes and diabetic retinopathy in type 2 diabetie patients. Methods Eighty - two eyes in 41 type 2 diabetie patients with diabetie retinopathy were examined by eolor Doppler imaging to investigate hemodynamie changes of ophthalmic artery (OA), central retinal artery (CRA) and posterior ciliary artery (PCA). Slit- lamp, ophthalmoscope and fundns fluoreseein angiography examination were applied on all the patients , and the patients were divided into NPDR group, PDR group and NDR group aeeording to the ocular fundi ehanges. The results were eompared with those of the normal control group. Results The peak systolic velocity (PSV), end diastolic velocity (EDV) and VM of OA, CRA and PCA in type 2 diabetie patients with diabetie retinopathy were lower than those of the normal eontrol group while RI inereased. The differenee between NPDR group , NDR group and eontrol group was signifieant ( P 〈 0.05) while the differenee between PDR group and eontrol group more significant ( P 〈 0.01 ). Conclusion There is a close relationship between ocular hemodynamie changes and the development of diabetie retinopathy in type 2 diabetic patients. Color Doppler imaging ean provide valuable information about retinal perfusion and retinal function ehanges, and it is also sign/fieant for evaluating degrees of diabetic retinopathy.
