摘要
目的探讨肺癌患者血清中恶性肿瘤相关物质TSGF、NSE和CYFRA21-1的水平及其临床意义。方法用电化学发光免疫法(ECLIA)检测210例肺癌患者、44例肺良性疾病患者及112例健康对照血清NSE和CYFRA21-1水平,同时用光电比色法检测其血清TSGF水平。结果与肺良性疾病组及健康对照组相比,肺癌患者血清中3项标志物的水平明显升高(P<0.05)。3项标志物的水平和敏感性与病理类型、非小细胞肺癌(NSCLC)临床分期及淋巴结转移有关(P<0.05)。TSGF、NSE及CYFRA21-1判别肺癌血清的敏感性分别为60.0%、38.1%和59.5%,特异性分别为92.9%、93.6%和96.8%,3项标志物联合检测判别肺癌血清的敏感性和特异性分别为87.6%和84.0%。治疗前及治疗后无效组患者血清3项标志物的水平均明显高于治疗后有效组(P<0.05)。结论血清TSGF、NSE及CYFRA21-1联合检测在肺癌诊断、病情监测及疗效判断方面具有较重要的临床价值,三者联合检测可明显提高肺癌诊断的敏感性。
[Objective] To detect the clinical significance of serum tumor supplies group of factors (TSGF), NSE and CYFRA21-1 in patients with lung cancer, [Methods] The serum levels of NSE, CYFRA21-1 and TSGF in 210 patients with lung cancer, 44 patients with benign lung disease and 112 healthy controls were measured by Elctrochemiluminescence immuno-assay (ECLIA) and spectrophotometry, [Results] The serum levels of TSGF,NSE and CYFRA21-1 in patients with lung cancer were significantly higher than those in benign lung disease and healthy controls (P 〈0.05), The serum levels and sensitivity of 3 tumor markers in patients with lung cancer were related to types of pathology, and the serum levels of 3 tumor markers in patients with non-small cell lung cancer were correlated significantly with clinical stage and lymph node metastasis. The sensitivity for lung cancer detection of serum TSGF, NSE and CYFRA21-1 were 60,0%, 38,1% and 59,5%, The specificity of TSGF, NSE and CYFRA21-1 were 92,9%, 93,6% and 96.8%, The sensitivity and specificity for lung cancer detection of combined detection of TSGF, NSE and CYFRA21-1 were 87,6% and 84.0%, The serum levels of 3 tumor markers for newly diagnosed cancers and non-responders were significantly higher than that for responders (P 〈0.05), [Conclusion] The combined detection of serum TSGF, NSE and CYFRA21-1 is of important clinical value in clinical diagnosis, monitor and evaluation of response to treatment of lung cancer, and the combined detection of serum TSGF, NSE and CYFRA21-1 might significantly improve the sensitivity for lung cancer detection.
