摘要
目的探讨影响社区脑卒中患者生活质量的相关因素。方法将304例社区脑卒中患者按随机数字表随机分成对照组153例和康复组151例。所有患者均予为期5个月的前瞻性随访研究，期间康复组给予定期随访、功能评定和社区康复治疗，对照组仅作同期的随访和功能评定，不作康复治疗。以生活质量测定量表简表（WHOQOL—BREF）中文版为评定量表，以患者生活质量改善幅度（用5个月末WHOQOL-BREF评分减去入组时WHOQOL—BREF评分表示）为观察终点来评价其生活质量，依次通过单因素分析和多元逐步回归分析，最终筛选出社区脑卒中患者生活质量的主要相关因素。结果患者入组情况、年龄组别、入组时WHOQOL-BREF评分、入组时功能综合评定（FCA）评分、病程、高血压、吸烟及高脂肪饮食与其生活质量有不同程度的相关性。结论早期系统化社区康复治疗可改善社区脑卒中患者的生活质量，控制血压、戒烟、低盐低脂饮食有利于康复进程。
Objective To identify the factors influencing the quality of life （QOL） of stroke patients in community. Methods A total of 304 stroke patients living in the community were randomly divided into the control group （ n = 153） and rehabilitation group （ n = 151 ）. All the patients were prospectively followed-up for 5 months, the patients of the rehabilitation group were treated with regular following-up, functional assessment and communitybased rehabilitation （CBR） , while the control group only treated with homochronous following-up and functional assessment, but no rehabilitation. The Chinese version of WHOQOL-BREF was used to rate the QOL. The improvement of QOL was the observational ending point for all patients, and it was apply to assess their QOL. Single factor analysis and multielement stepwise regression analysis were used to identify the main factors influencing the QOL of stroke patients. Results The QOL of the stroke patients is related with their initial condition, age, the course of disease, the initial WHOQOL-BREF scores and functional comprehensive assessment （FCA） scores, and hypertension, smoking, high fat diet or not. Conclusion Earlier systematized community-based rehabilitation can improve the QOL for stroke patients living in the community, and normal blood pressure, smoke abatement, diet with low salt and low fat were beneficial to their recovery.
