摘要
目的观察甘露醇对大中型皮瓣术后早期肿胀的疗效。方法30例大中型皮瓣术后给予20%甘露醇治疗一周(A组),选择术后给予七叶皂甙钠治疗者30例作为对照(B组),并设立空白对照30例(C组),每日两次记录皮瓣所在肢体的肿胀状况。对比三组患者肿胀的变化情况及消肿时间。结果A组轻度肿胀8例,中度肿胀18例,重度肿胀4例。显效20例,有效9例,无效1例,总有效率达96.7%。未见有严重药物毒副作用发生。肿胀程度较C组明显减轻(P<0.05)。A组平均消肿时间少于B组(P<0.05)。结论采用甘露醇治疗皮瓣术后早期肿胀,具有简便易行、快速消肿、安全经济等优点,值得临床推广应用。
Objective To observe effects of mannitol on prevention of postoperative swelling in large and middle free flaps.Methods A series Of 90 clinical medium or large flap cases were selected and divided into 3 groups. In Group A（n=30） 20% Mannitol was given for one week. In Group B（n= 30）sodium aescinate was given. In Group C（n=30）, control group,no drug was given for preventing swelling. Swelling of laps in all the cases were measured and observed twice daily with the same criteria. The subsidence time was recorded.Results In Group A,light swelling occurred in 8 cases, middle swelling in 18 cases and serious swelling in 4 cases. No serious complication occurred in all patients. The results in Group A were found to be good in 20 cases, fair in 9 and poor in 1. The overall effective rate was 96.7%. The degree of swelling in Group A was lighter than Group C and the average time of subsidence in Group A was less to Group B. Conclusion Mannitol treatment is an effective, quick, safe and cheap method in preventing and managing postoperative swelling of flaps. It could be widely used in clinic.
