肾肿瘤中浸润性树突状细胞的免疫状态变化

The change of immune state of tumor infiltrating dendritic cells in renal cell carcinoma

摘要 目的通过分析肾肿瘤中浸润性树突状细胞的免疫状态变化,以探讨其在肾肿瘤微环境中的功能状态。方法选择未经任何非手术治疗的肾透明细胞癌患者标本40例作为实验组,10例正常肾组织作为对照组,通过免疫组织化学技术标记组织中树突状细胞(dendritic cell,DC)的CD1a、HLA-DR和CD86抗原,观察正常肾及肾透明细胞癌组织中DC的3种抗原的表达,结果进行统计学分析。结果所有肾透明细胞癌组织中均未见明显CD86+DC,但均有CD1a+DC,其在肾癌组织中的浸润程度明显高于正常肾组织(P<0.001),但CD1a+DC与肾癌临床分期及组织学分级均无相关性;在肾癌组中有34例癌实质内的DC表达HLA-DR抗原,其阳性表达率为85%,与正常肾组织80%的阳性表达率相比无统计学意义(P>0.05)。结论DC具有趋化性,可以向肿瘤组织聚集,肾癌组织中存在CD1a,肾癌患者CD86表达低下,提示肾癌患者DC存在免疫缺陷,而DC的免疫功能低下则可能是肾癌逃避宿主免疫监视的主要原因。 Objective To elucidate the functional status of dendritic cells (DCs) in the tissue of renal cell carcinoma (RCC). Methods Forty specimens from RCC cases primarily treated with surgery were selected as test group. In addition, 10 specimens of normal kidney tissue were used as control. Distributions of DC expression CDla+ , HLA-DR+ and CD86+ in the tumor tissues and normal tissues were observed by immunohistochemistry. The density of DC and the rate of HLA-DR in renal cell carcinoma and control groups were statistically compared. Results There was no CD86+ DC in all cancers, but CDla+ DC was found in all cancers. The density of CDla+ DC in tumor tissues was significantly higher than that in normal tissues (P〈0.05). But CDla+ DC had no relevance with the grade or the stage of the cancers. HLA-DR antigen expressed on the surface of DC in 34 tumoral tissues and 8 normal tissues. There was no statistic significance between the rates of HLA-DR posi- tive expression of the two groups. Oonclusion DC has the tendency to gather in tumor tissues. There is CDla+ DC in RCC tissues, but no CD86+ DC in all cancers, indicating the functional nature of DC might be inhibited by the immunosuppressive action of RCC. The lower immune state of DC may contribute to RCC escaping from immunosurveillance.
