食管癌中MMP-9蛋白表达及其临床意义 被引量:6

MMP-9 PROTEIN EXPRESSION IN ESOPHAGEAL SQUAMOUS CELL CARCINOMA AND ITS CLINICAL SIGNIFIENCE

摘要 目的探讨食管癌中基质金属蛋白酶-9(matrix metalloproteinase-9,MMP-9)蛋白表达与临床病理因素间的关系及其对单纯放射治疗(放疗)预后的影响。方法应用免疫组织化学SP法检测59例食管癌和41例食管癌旁组织中MMP-9蛋白的表达情况,分析该蛋白表达与临床病理因素间的关系及其对食管癌单纯放疗预后的影响。结果59例食管癌组织中MMP-9蛋白阳性表达率为84.75%,41例癌旁组织中阳性表达率为39.02%(P=0.000)。胸部CT扫描显示病变外侵深度>2 cm者MMP-9蛋白表达阳性率为100%,明显高于外侵深度≤2 cm者的78.57%(P=0.048)。进一步分析发现,MMP-9蛋白表达与食管癌单纯放疗的疗效和预后未见明显相关性(P>0.05)。结论MMP-9蛋白高表达可能与食管癌侵袭有关,但不能用于食管癌放疗疗效评价。 Objective To explore the relationship between the MMP-9 protein expression and clinical and pathologic factors and to evaluation the putative influence on prognosis of the patients with esophageal squamous cell carcinoma treated by radiotherapy. Methods MMP-9 protein expression in 59 cases of esophageal carcinoma tissues and 41 cases of dysplasia adjacent to tumor tissues were studied with immunohistochemistry. The relationship between expression of MMP-9 protein and the clinicopathological parameters was analyzed. The prognostic factors were studied in patients with esophageal squamous cell carcinoma treated by radiotherapy alone. Results The positive expression rates of MMP-9 protein in specimens of esophageal carcinoma and corresponding dysplasia tissues were 84.75% and 39.02% respectively (P 〈 0.01). The expression of MMP-9 protein was significantly correlated with the depth of tumor invasion in CT image. The positive expression rates of MMP-9 protein was 78.57% in patients with the depth of tumor invasion no more than 2 cm and 100% in patients with that than 2 cm. The protein expression of MMP-9 did not significantly affect the prognosis and the short-term result of esophageal carcinoma treated by radiotherapy. Conclusion Protein expression of MMP-9 was probably correlated with local invasion of esophageal carcinoma, it probably plays an important role in tumor development.
