摘要
目的:通过检测临床疑为强直性脊柱炎(AS)患者的HLA-B27表达,探讨HLA-B27抗原的检测在AS早期诊断中的意义。方法:采用流式细胞术对1807例疑为AS患者外周血进行HLA-B27表达的检测。结果:在867例诊断为AS的患者中,HLA-B27阳性758例,HLA-B27检测AS的敏感性为87.4%(758/867),特异性为91.9%(864/940),诊断正确率为89.8%。结论:AS与HLA-B27阳性高度相关,临床疑为AS的患者若加上流式HLA-B27抗原检测结果将对AS的早期诊断起到重要作用。
Objective To evaluate the significance of human leukocyte antigen （HLA-B27） expression in the early diagnosis of ankylosing spondylitis（AS）. Methods A flow cytometric assay was used to detect lymphocyte HLA- B27 expression in 1807 patients with presumed AS. Result The positive rate of HLA-B27 expression was 87.4% （758/ 867） in patients with AS and 8.1%（76/ 940） in patients without AS, which indicated that the sensitivity of the method was 87.4% the specificity was 91.9% and the accuracy of diagnosis was 89.8%. Conclusion There is positive correlation between AS and HLA-B27 expression. The flow cytometric assay of HLA-B27 expression is a convenient, useful screening test for the early diagnosis of AS.
