摘要
目的探讨妊娠合并血栓性血小板减少性紫癜的诊断和治疗。方法回顾分析我院救治成功的2例妊娠合并血栓性血小板减少性紫癜的资料。结果2例患者均出现溶血性贫血、血小板减少、神经系统异常、发热、肾脏、肝脏、心脏等多器官系统损害,起病初均未能正确诊断;经行血浆置换、血液透析及支持对症治疗,救治成功。结论妊娠合并血栓性血小板减少性紫癜可出现多器官功能损害,易误诊;诊断血栓性血小板减少性紫癜时依据微血管性溶血性贫血、血小板减少、神经系统异常、发热及肾脏损害;治疗上采取以血浆置换为中心的综合治疗。
Objective To explore the diagnosis and treatment of thrombotic thrombocytopenic purpura in pregnancy.Methods The data of 2 patients of thrombotic thrombocytopenic purpura in pregnancy who were cured successfully in our hospital were looked back to.Results Microangiopathichemolytic anemia,thrombocytopenia,neurological abnormalities,fever,harm to multi-organ such as kidney,liver and heart took place in both cases.Both cases cannot be diagnosed correctly at first,and were cured successfully by plasma exchange,blood modialysis,supportive therapy and allopathy.Conclusion Thrombotic thrombocytopenic purpura may harm to multi-organ and is prone to be mis-diagnosed,diagnosis can be made based on microangiopathichemolytic anemia,thrombocytopenia,neurological abnormalities,fever and harm to kidney.Integrative treatment with plasma exchange in the core should be adopted.
