小剂量激素辅治大叶受累的肺炎支原体肺炎临床观察 被引量:13

摘要 目的:观察小剂量糖皮质激素辅助治疗小儿大叶受累的肺炎支原体肺炎(MPP)的疗效。方法:将MPP胸部X线(胸片)表现为大叶受累的患儿64例随机分为2组,均给予常规治疗,治疗组在此基础上给予地塞米松[0.2mg/(kg.d)]静滴,连用3~5d,对比观察两组退热、胸片大片阴影消退时间、C-反应蛋白(CRP)变化及住院时间。结果:治疗组发热持续时间、胸片大片影消退时间及住院时间明显少于对照组(P<0.05),且CRP水平随病情好转显著下降,差异有统计学意义(P<0.05)。结论:小剂量激素辅助治疗小儿大叶受累的MPP,可有效的缓解临床症状,缩短发热和胸部大片影消退时间,从而减少住院天数。 Objective: To evaluate the effectiveness of low doses of glucocorticoid in the assistance treatment of mycoplasmal pneumoniae lobar pneumonia in children.Methods: Sixty-four cases of mycoplasmal pneumonia were divided into two groups according to chest X rays manifested of lobal pneumonia. Thirty-two cases in control group were given routine treatment and thirty-two cases in observation group were add the dexamethasone intravenously for 3 -5 days. The level of serum c-reactive protein (CRP), duration of fever, regression of lobar shadow and hospital time of two groups were observed. Results: There were significant differences between the two groups in duration of fever, regression of lobar shadow and hospital time (P〈0.05). The level of serum CRP decreased with the restore of disease. Conclusion: Adjunctive therapy with low doses of glucocorticoid in treating mycoplasmal lobar pneumonia could effectively alleviate the clinical symptoms,decrease the duration of fever restore of lobar shadow, and reduce the hospital time.
