摘要
目的确定经皮髂骨拉力螺钉固定术治疗髂骨后部骨折钉道角度。方法采用多层螺旋CT对56例正常成人进行骨盆三维重建,以双侧髂后上棘与髋臼上缘连线平面为起始截面,以髂后上棘为中心,自起始截面以0.5°为单位向头侧旋转行斜位断层图像重建,测量每一截面髂骨皮质距正中线的最短距离,>4 mm为可进针截面,直至≤4 mm的终止截面,终止截面上中线与矢状线的夹角即为进针(钉道)角度。结果起始角度为(38.13±0.94)°,终止角度为(23.63±1.17)°,进针角度为(24.68±1.06),°且该角度无性别差异。结论钉道角度相对恒定,在此范围内进针安全、有效。
Objective To determine the passage's angles of per cutem iliac screw for the treatment of iliac bone fractures. Methods 56 Chinese adults's ilia were reconstracted by multilayer helico-CT. The beginning loxosis fault was defined by linking the both line from posterior superior iliac spine to top of acetabular bone, and the posterior superior iliac spine was taken as the center, the oblique fault images was reconstructed since the beginning loxosis fault to cephalic one at 0.5° as the rotation units, the shortest distance between ilium cortex and midline from each fault was measured, the fault with the distance exceed 4mm was selected as the available ones and the one with the distance less than 4mm as the ending faults. The angle between the mldline of ending fault and sagittal line was the angle of passage. Results The beginning angle was(38. 13 ±0. 94)° and the ending one was (23. 63± 1.17)°, the angle of needling was (24. 68 ± 1.06)° with no gender differences. Conclusions The angle of passage is relatively constant and is safe and effective for needling.
