肾移植急性排斥反应时Bcl-2/Bax及Fas/FasL蛋白表达的变化 被引量:6

摘要 背景:在器官移植急性排斥反应过程中,靶细胞的凋亡被认为是移植物功能丧失的主要作用途径之一。细胞凋亡受一系列分子调控,其中Bcl-2家族起着决定性作用;Fas系统介导的细胞凋亡亦在移植免疫中起十分重要的作用。目的:分析移植肾发生急性排斥反应时Bcl-2/Bax,Fas/FasL蛋白表达规律与急性排斥反应预后的关联。设计、时间及地点:观察对比实验,于2002/2005在解放军总医院第二附属医院器官移植中心完成。对象:收集肾移植术后发生急性排斥反应的移植肾穿刺活检蜡块标本30例;另选同期20例无明显排斥反应表现肾移植患者的移植肾穿刺活检蜡块标本作为对照组。方法:应用免疫组织化学方法检测移植肾急性排斥反应穿刺活检标本中Bcl-2/Bax,Fas/FasL蛋白的表达水平,并与对照组进行对比。主要观察指标:急性排斥组和对照组移植肾穿刺活检标本中Bcl-2/Bax,Fas/FasL蛋白的表达水平。结果:①Bcl-2/Bax,Fas/FasL蛋白主要在肾小管上皮细胞中表达。②两组肾小管上皮细胞中Bcl-2表达水平差异无显著性意义(P>0.05),急性排斥反应组肾小管上皮细胞中Bax,Fas及FasL蛋白表达水平均高于对照组(P<0.05)。结论:在肾移植急性排斥反应中Bcl-2家族可能参与调控细胞凋亡,引起移植肾免疫损伤;Fas及FasL可能直接参与移植肾急性排斥反应,造成肾小管上皮细胞凋亡,引起移植肾免疫损伤。 BACKGROUND: In acute rejection after organ transplantation, apoptosis of target cells is considered one of action pathways of transplant functional incapacitation. Cell apoptosis is regulated by a series of molecules, in which Bcl-2 family exhibits an important effect. Fas system mediated cell apoptosis also plays a crucial effect on transplantation immunity. OBJECTIVE: To analyze the correlation of Bcl-2/Bax, Fas/FasL protein expression rule in transplanted kidney with acute rejection to the prognosis of acute rejection. DESIGN, TIME AND SETTING: The control experiment was performed at the Center of Organ Transplantation, Second Affiliated Hospital, General Hospital of Chinese PLA from 2002 to 2005. PARTICIPANTS: Thirty paraffin-embedded samples of transplanted renal needle biopsy with acute rejection following renal transplantation were enrolled, and an additional twenty paraffin-embedded samples of transplanted renal needle biopsy without significant acute rejection were selected as controls. METHODS: Bcl-2/Bax, Fas/FasL protein expression in samples of transplanted renal needle biopsy with acute rejection was measured by immunohistochemistry. The results were compared with that in the control group. MAIN OUTCOME MEASURES: Bcl-2/Bax, Fas/FasL protein expression in samples of transplanted renal needle biopsy in the acute rejection and control groups. RESULTS: Bcl-2/Bax, Fas/FasL protein expression was mainly in renal tubular epithelial cells. No significant difference in Bcl-2 expression was detected in renal tubular epithelial cells in both groups (P 〉 0.05). Bax, Fas and FasL protein expression was higher in the acute rejection group than in the control group (P 〈 0.05). CONCLUSION: Bcl-2 family maybe participate cell apoptosis and induce immunologic injury in transplanted kidney acute rejection after renal transplantation. Fas and FasL maybe participate in acute rejection, which can result in apoptosis of renal tubular epithelia cells and immunologic injury of transplanted kidney.
