中度寻常痤疮瘢痕形成危险因素非条件logistic回归分析 被引量:5

摘要 目的应用非条件logistic回归分析,探讨中度寻常痤疮患者瘢痕形成危险因素,为预防和控制瘢痕形成提供依据。方法通过调查问卷调查、粉尘螨和屋尘螨变应原皮肤点刺试验(SPT)、蠕形螨和糠秕孢子菌显微镜检查,采用非条件logistic单因素和多因素逐步拟合法回归分析290例中度痤疮患者的性别、发病年龄、饮食、皮损发生部位、皮肤类型、文化程度、伴发脂溢性皮炎、酒渣鼻、蠕形螨和糠秕孢子菌寄生、尘螨SPT阳性和家族史等因素对中度痤疮丘疹和脓疱后瘢痕形成的影响。结果经多因素logistic回归模型分析显示,中度寻常痤疮的炎性丘疹及脓疱消退后瘢痕形成与性别、皮损发生部位、家族史、伴发脂溢性皮炎、蠕形螨寄生和尘螨SPT阳性密切相关,危险程度由大至小依次为伴发脂溢性皮炎(OR=13.051)、尘螨SPT阳性(OR=9.327)、男性(OR=6.243)、家族史(OR=4.769)、蠕形螨寄生(OR=4.003)。瘢痕形成主要发生于颊部(OR=3.400)。结论男性、有痤疮瘢痕家族史,伴发脂溢性皮炎和有蠕形螨寄生及尘螨SPT阳性的痤疮患者易形成瘢痕,痤疮患者应常规行尘螨变应原点刺试验和蠕形螨等检查,积极争取早期治疗相关疾病,有利于降低瘢痕形成的风险。 Objective To evaluate the risk factors of mid-grade acne vulgaris prone to scar using non-conditional logistic regression. Methods A questionnaires survey and clinic examination were conducted in 290 patients with mid-grade acne vulgaris,the concerning factors including the age of onset,gender,educatlonal status,diet custom,the lesion's slte,family history,skin types,seborrheic dermatit,rosacea,the parasitism of the humand Demodex. Pityrosporum yeast and the SPT positive reaction to dust mite, Simple logistic regression analysis was wade for analysis of all the risk factors and then those statistically significant variables were chosen for multiple logistic regression analysis. Results The results of multiple logistic regression analysis showed that the statistically significant risk factors were seborrheic dermatit ( OR = 13. 051 ) ,the SPT positive reaction to dust mite (OR =9, 327) ,male(OR =6.243) ,family history ( OR =2,904), patients had scar on cheek(OR =3.400), the parasitism of the humand Demodex(OR =4, 003), Conclusion Factors,including seborrheic dermatit , male, heredity, the lesion's site of cheek and the parasitism of the humand Demodex play important roles in the pathohogenesis of scar in mid-grade acne vulgaris,these risk factors should he controled for patients with ache vulgaris.
