血管内皮细胞与加速血管内支架内皮化的进程 被引量:5

摘要 血管内再狭窄是血管对抗损伤的一种异常修复反应,表现为支架植入后血管的收缩性重构,局部的炎症与血栓形成,以及以血管平滑肌细胞增殖为主的内膜增生。血管内皮通过合成和分泌活性物质来调节和维持血管的收缩与舒张平衡、维持凝血和纤溶系统之间的动态平衡、维持血管的增殖平衡、控制炎症反应的程度,对维持血管正常生理功能具有重要的作用。支架置入后血管内皮功能异常,引发内皮层剥脱,导致血小板聚集、炎症反应、血管平滑肌细胞增殖迁移、细胞外基质形成等一系列病理反应,造成支架内再狭窄,因此促进血管内皮细胞损伤、修复及功能改变在支架内再狭窄中起着重要作用。目前,提高血管内皮细胞与支架的黏附率,寻找更有特异性的血管内皮细胞表面标记,是加速支架植入后血管内支架内皮化进程需要解决的主要问题。 Endovascular restenosis is an abnormal repairing response of vessel against injury. Following stent implantation, vessel contractibility reconstitution, local inflammation and thrombogenesis, intima hyperplasia, mainly vascular smooth muscle hyperplasia, are found. Blood vessel endothelium mediates and maintains vascular construction and relaxation balance through active mass synthesis and secretion, maintains coagulation and fibrinolytic system balance and vessel proliferation balance, controls inflammatory reaction degree, and maintains vascular normal function. Stent implantation induces abnormal function of blood vessel endothelium, resulting in platelet aggregation, inflammatory reaction, vascular smooth muscle proliferation and migration, and extracellular matrix formation, which causes intra-stent restenosis. Therefore, to promote vascular endothelial cell injury, repair and functional alterations is important in intra-stent restenosis. To date, how to improve attachment of vascular endothelial cell and stent, and to search specific vascular endothelial cell surface marker are primary problems to accelerate endothelialization following endovascular stent implantation.
