摘要
目的探讨适合北京地区脑卒中患者的DRGs病例组合方案。方法以AP-DRGs v18.0为基础,根据影响北京地区脑卒中住院费用的影响因素以及该病的临床特点调整分组路径,按决策树的方法将脑卒中病例分入相应的DRGs组,并与AP-DRGs v18.0脑卒中病例组合方案的分类结果进行比较。结果调整后的病例组合方案将病例分入12个相应的DRGs组,组间住院总费用对数值差异有统计学意义,P<0.0001,R2=0.3106,未分入组病人2116人。结论相对于国际上现行的AP-DRGs方法,修改路径以后形成的DRGs组,其组内同质性和组间异质性均有增加,未分入组病例数也有所减少,经调整后的新的DRGs分组方法更适合北京地区。
Objective To explore an appropriate stroke case mixed method of Diagnosis Related Groups（DRGs） in Beijing. Methods According to the factors affecting stroke hospitalization costs, AP-DRGs v18.0 was adjusted. The stroke inpatients were reclassified with the new method. Results With the adjusted DRGs group method, the difference of logarithmic total hospitalization costs among the new twelve groups is significant（P〈 0.001）, R^2 = 0.3106. Conclusion Compared with AP-DRGs v18.0, the adjusted method is more suitable for the situation in Beijing.
