摘要
目的探讨原位肝移植术后经鼻胃管早期规范化肠内营养监护措施的应用及效果,以便更好地进行手术后护理工作。方法对316例成人原位肝移植术后早期经鼻胃管行肠内营养的患者采取规范化的肠内营养监护措施,强化导管护理、患者体位的管理、胃肠道并发症的监测与血糖监控。结果316例经鼻胃管行肠内营养的患者中,3例由于全身状况恶化、腹内压增高而终止肠内营养,其余均过渡到完全经口进食,平均肠内营养时间(82.6±7.5)h,未见吸入性肺炎的发生。结论规范化肠内营养监测可保障肠内营养的顺利进行,促进肝脏及各脏器功能的恢复,减少并发症的发生。
Objective To evaluate the effect of standardization monitoring in early enteral nutrition(EN) after orthotopic liver transplantation. Methods Including care of nasal gstric tube,patients' position,complication of gastrointestinal tract and controlling glucose,Standardization monitoring were performed to 316 patients after orthotopic liver tansplantation. Results All patients completed EN and took food by mouth except three terminated because multiple organ disfunction and raised intra-abdominal pressure.The average time of early enteral nutrition was (82.6±7.5)h. There was not the complication of aspiration pneumonia.Concluslon Standardization monitoring can ensure the conducting of enteral nutrition,accelerate the recovery of liver function and reduce the happening of complication.
